for commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-15. This procurement
is unrestricted. The standard industrial classification (SIC) code is
8099, Health and Allied Services, N.E.C. The small business size
standard is $5 million average annual receipts for the past three
years. Offerors are encouraged to submit quotes based on an estimated
1,040 hours. Proposed rates should include all costs associated with
providing the following services. The Lower Michigan Acquisition
Center, Detroit, Michigan, has a requirement for a board certified
general internist, Request For Proposals (RFP) 553-38-00, 5 days per
week, 8:00a.m. to 4:30 p.m., from February 21, 2000, through August 20,
2000, at the John D. Dingell VA Medical Center, 4646 John R. Street,
Detroit, Michigan. A provider is needed on-site no later than February
21, 2000. The contract physician shall primarily conduct Compensation
and Pension (C&P) examinations, order and interpret tests, and dictate
results using the Veterans Health Information Systems and Technology
Architecture (VISTA)/Decentralized Hospital Computer Program (DHCP). At
a minimum individuals proposed to work under this contract shall: (1)
be board certified in Internal Medicine; and (2) possess a full and
unrestricted license to practice medicine in any state, territory or
Commonwealth of the United States, or the District of Columbia.
Individuals proposed to work under this contract should have experience
performing C&P examinations and shall be required utilize computer
keyboarding skills and medical records software programs. The contract
physician shall be required to complete approximately 40 C&P exams per
week and shall, when requested, review approximately 15 medical records
per month and provide expert opinion on each record reviewed. The
contract physician will be required to observe all Medical Center
policies and memorandums. The provision at 52.212-1, Instructions to
Offerors Commercial (NOV 1999), applies to this acquisition. The
provision at 52.212-2, Evaluation -- Commercial Items (JAN 1999),
applies to this procurement. Evaluation criteria including in paragraph
a are, in descending order of importance, (1) Experience performing
compensation and pension examinations, (2) the contractor's past
performance, and (3) price. Experience and Past Performance are
significantly more important than Price. Offerors are required to
include a completed copy of the provision at 52.212.3, Offeror
Representations and Certifications -- Commercial Items ALT III (JAN
1999), with its offer. The clause at 52.212-4, Contract Terms and
Conditions -- Commercial Items (MAY 1999), applies to this acquisition
