Contract period will include a base period, not to exceed 6 months, and four 6 month option periods for renewal. Requirements will average 22 hours per week and shall not exceed 30 hours per week. Award will be made to the lowest offeror who meets the following qualifications: graduated from an accredited dental hygiene program and possesses an associate or bachelor's degree in dental hygiene; holds a valid dental hygienist license issued by any state; is in good standing in the healthcare community as evidenced by references provided by the offeror. NOTE: The awardee will be required to carry medical liability insurance and shall provide evidence of coverage prior to award. The RFQ will be available on February 8, 2000, and the date and time for receipt of quotes is scheduled for 1:00pm on February 18, 2000.
