Contract period will include a base period, not to exceed 6 months, and four 6 month option periods for renewal. Requirements will average 26 hours per week and shall not exceed 30 hours per week. Award will be made to the lowest offeror who meets the following qualifications: graduated from an accredited dental assistant program or has adequate on the job training to perform the tasks routinely required of a dental assistant; holds a valid dental x-ray permit issued by the Commonwealth of Pennsylvania; is in good standing in the healthcare community as evidenced by references provided by the offeror. NOTE: Two letters of recommendation will be required from dental care providers that are knowledgeable of the offeror's work practices. The RFQ will be available on February 8, 2000, and the date and time for receipt of quotes is scheduled for 1:00pm on February 18, 2000.
