Q -- COMMUNITY BASED OUTPATIENT CLINICS SOL RFP 544-032-00 DUE 040400 POC Brenda Tabor, Contract Specialist, (803) 695-6846 Contractor shall provide Primary Medical Care Services at their facilities, to beneficiaries of the WJBD VA Medical Center. The Contractor shall provide the following services: (a) Clinical Office Space and Designated Parking Spaces, (b) Clinical Services, (c) Security Services, (d) Janitorial Services, (e) Maintenance and Lawn Services (if applicable), (f) Laboratory Services, (g) Radiology Services, (h) Snow Removal Services (if applicable), (i) Laundry Services, (j) Coding and Compliance Services, and (k) Maintenance Service on Government Furnished Biomedical Equipment that fullfill the VA's healthcare delivery requirements, in accordance with all terms, conditions, and provisions of the solicitation. It is the VA's intention to solicit proposals and possibly award multiple contracts on a competitive basis. Veterans receiving services under this contract(s) reside primarily in the following catchment areas for each of the following Community Based Outpatient Clinics (CBOC): (1) Florence, SC Consisting of Florence, Darlington, Marion, Chesterfield, Dillion Marlboro, and Williamsburg Counties. (2) Orangeburg, SC Consisting of Orangebur, Allendale, Barnwell, Calhoun, and Bamberg Counties. (3) Sumter, SC Consisting of Sumter, Clarendon, and Lee Counties. (4) Anderson, SC Consisting of Oconee, Pickens, Anderson, Abbeville, and Greenwood Counties. The CBOC Contractor(s) facility or facilites to provide services must be physically located in the city limits referenced above for each individual CBOC. The offeror/basic provider will be expected to sub-contract for any required service, that it does not provide, at no additional cost to the Government. The contract(s) includes Base Year (one year from date of award) plus 4 one-year renewal options that may be exercised at the option of the Government. The total duration of the contract(s) will not exceed 5 years. This solicitation is issued as a commercial item in accordance with FAR Part 12 and shall be evaluated in accordance with FAR Part 15. Request for Proposal RFP 544-032-00 is advertised as full and open competition with SIC: 8011 with the size standard of $5.0 million. The solicitation will be issued o/a 3/1/2000. The closing date for submissions of proposals is 4/4/2000 at 3:00 pm. local time.
