This is a combined synopsis/solicitation for commercial items prepared in accordance with format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. RFP-797-NC-00-0032 is issued as an request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-15. This proposed contract is 100% set aside for small business concerns. SIC code is 3089 and the small business size is less than 500 employees. The National Acquisition Center requires annually 4,366 Body bags:plastic, white opaque color, leak and water proof, 8 mil thick, overall dimension of at least 40"x96", ID tag, attached string 2 ties and 1 chin strap. Delivery shall be FOB destination within 15 business days of order, as required by any of the 172 Dept. of Veterans Affairs Medical Centers (VAMC) nationwide including Alaska, Hawaii and Puerto Rico. The contract will be for one base year and 4 option years. Estimated annual sales are $42,499.00. Award will be based on aggregate pricing for all 5 years.
