Conduct survey of VA clinical facilities in the states of Connecticut, Maine, Massachusetts, New Hampshire, Rhode Island, and Vermont to determine compliance with NFPA 101, Life Safety Code. Deviations from NFPA 101 will be identified and listed by type so that a Plan for Improvement can be developed. The following data will be presented in the four-part JCAHO format for tracking status in a database. Part I Introduction, Part II -- Basic Building Information, Part III -- Life Safety Assessment, Part IV -- Plan for Improvement forms. All work shall be completed by April 20, 2000. Architect-Engineer firms which meet the requirements in this announcement are invited to submit: (1) a Standard Form 254, Architect-Engineer and Related Services Questionnaire; (2) a Standard Form 255, Architect-Engineer and Related Services Questionnaire for Specific Project. The firm must demonstrate its qualifications with regard to these evaluation factors: (1) Background of team proposed for this project; (2) Previous experience of team proposed for this project; and (3) Capability of meeting project schedule.
