a combined synopsis/solicitation for commerical items prepared IAW the
format in Fed. Acquisition Reg. Subpart 12.6, as supplemented
w/additional information included in this notice. This notice
constitutes the solicitation; quotes are being requested; separate
solicitation document won't be issued. (ii) Solicitation number is
DE-RQ08-00NV13811, REQUEST FOR QUOTATION (RFQ). The solicitation
document and incorporated provisions/clauses are those in effect
through Fed. Acquisition Circular 97-14. (iv) This requirement is 100%
set-aside for small businesses. Standard Industrial Classification
Code is 7623. Size standard is $5M (avg. of total sales over 3 years
can't exceed the size standard). (v) Offeror shall provide all
materials, labor, and transportation necessary to provide 12 months of
preventative maintenance for the refrigerant storage and recycling
systems for the Nevada Support Facility (NSF), 232 Energy Way, N. Las
Vegas, NV 89030. (vi)Following is the statement of work (SOW) for
preventative maintenance: The vendor shall perform preventative
maintenance for the refrigerant storage and recycling systems for the
NSF central plant beginning March 1, 2000 and continuing through
February 28, 2001. The vendor will provide a complete program of
preventative maintenance on three York rotary screw water chillers,
model number YSCACAS2-CFB, rated at 200 tons and three York solid state
motor starters, model number 7L. The equipment is to be maintained in
good working order, and all preventative maintenance tasks shall be
performed by factory-trained technicians in accordance with the
guidelines issued by York Engineering for the model(s) specified. The
preventative maintenance program must meet the manufacturer's
requirements to properly maintain and service the equipment and will
consist of (1) a minimum of five [5] operating inspections; (2) an
annual shut down to evaluate equipment status; (3) brush clean
condenser tubes; (4) an oil analysis for each unit; (5) oil and oil
filter changes as required; (6)fine tuning and adjustment for optimum
operation; (7) operator training; and (8) provide 24 hr, 7 days a week
emergency response as needed. When performing Item (5), should
refrigerant charge be removed, stored or recharged, the vendor shall
provide storage and recycling equipment and perform these activities
with technicians who have been trained and certified. All disposal of
used oil and filters will be done by the vendor per EPA guidelines and
written confirmation will be provided to DOE. The vendor shall provide
a written report of findings, corrective actions, and recommendations
following each visit. The NSF is a secured facility and requires a
DOE/NV sign-in and/or escort for any work to be done. Coordination
w/point of contact (POC) is required prior to arrival to perform work
to avoid delays in gaining access to the NSF. If access is denied due
to failure to coordinate arrival w/POC, the Gov't won't be liable for
costs of that trip. (vii) Delivery Point is NSF, 232 Energy Way, N. Las
Vegas,NV 89030. Acceptance is at destination. Delivery is per
instructions in para. (vi). NOTE: Following clauses are located at
website. (viii) FAR provision 52.212-1, Instructions to Offerors-Comc'l
Items, applies and the following addenda: para (bz)(4) In addressing
technical portion of RFQ, offeror must provide their plan to accomplish
work, schedule, and labor plan. (b)(11) An SF-1449 is to be completed
and signed (3 copies) and returned with offer. (ix) Refer to provision
at 52.212-2, Evaluation -- Comc'l Items: evaluation factors are
technical approach, past performance, and price. Tech. approach/past
perform., when combined, are equal to price. (x) Offeror is to complete
and return provision at 52.12-3, Offeror Reps & Certs -- Comc'l Items.
(xi) Clause 52.212.4, Contract Terms & Conditions -- Comc'l Items is
applicable. (xii) Clause 52.212-5, Contract Terms & Conditions Required
to Implement Statutes & Executive Orders -- Comc'l Items is applicable
