This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; Quotes are being requested and a
written solicitation will not be issued. Solicitation F45613 00BA007 is
issued as a request for quote. The solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 97-14. Standard Industrial Code is 5065. This
acquisition is 100% set-aside for small business, less than 100
employees. The contractor shall design and install an audio/visual
system for the newly constructed 96th Squadron Operations building. The
audio/visual system encompasses the following areas: 1. The squadron
auditorium, consisting of a 100 person theater type seating arrangement
with roughed in systems. There are various cutouts for equipment and
the projection screens are installed 2. The rear projectionroom, a
narrow rectangle type of construction with conduit connections and
electrical equipment stations 3. Training facility, consisting of a
classroom with an existing electrical pull down screen. The audio
visual equipment must conform to the room construction and layout 4.
The television monitoring system, consisting of various televisions,
ceiling mounted throughout the building and capable of cable, computer,
and other inputs as necessary 5. Mobile audio visual system consisting
of a VCR, portable projection system, laptop computer (highly graphics
capable), and wireless mouse/pointer combination. Upon completion of
the project, the contractor will provide full documentation of
equipment and training as necessary. The contractor will register all
equipment and system warranties on behalf of the government as
required. A listing of equipment warranties will be provided to the
project director. All warranty information will be included in the
quote at the time of submission. It is mandatory for all parties
interested in submitting a quote to attend a site visit on 31 Jan 00.
Contractors are to meet at the 92d Contracting Squadron, Fairchild AFB,
WA at 1:00 pm. At the site visit detailed information for installation
will be given to allow for accurate quotes from contractors.
Contractors that are unable to attend this site visit may call Lt.
Whittle to make other arrangements. Quotes shall include the following
price breakdown: 1) List of all items to be used and their prices. 2)
Installation fees. 3) Transportation Cost. 4) All warranty

The contractor shall be required to maintain Material Safety Data
Sheets (MSDS) for all hazardous materials in accordance with base and
federal guidelines. The contractor shall submit to the contracting
officer at the beginning of the contract a list of the hazardous
materials he is bringing on base for performance of the contract. This
list shall include all materials to be used by the prime contractor
and all subcontractors. The list will include estimated quantities of
each hazardous material that is contemplated to be used during the
course of the contract. The contractor will also provide a copy of the
MSDS for each product identified on the list. At the end of the
contract the contractor shall submit to the contracting officer a list
of products and the quantity of each that the contractor is taking off
base. The provision at FAR 52.212-1, Instructions to
offerors-commercial items (Nov 99) applies to this acquisition. The
provision at FAR 52.212-3, Offeror Representations and
Certifications_Commercial Items (Oct 99) applies to this acquisition.
A completed copy of this provision shall be submitted with any
quotation. The clause at FAR 52.212-4, Contract Terms and Conditions
(May 99) applies to this acquisition. The clause at FAR 52.212-5,
Contract Terms and conditions Required to Implement Statues of
Executive Orders-Commercial Items (May 99) applies to this acquisition;
under para (b), the following clauses are incorporated: FAR 52.222-26
Equal Opportunity (E.O. 11246), FAR 52.222-35 Affirmative Action for
Disabled Veterans and Veterans of the Vietnam Era (38 USC 4212), FAR
52.222-36, Affirmative Action for Workers with Disabilities (29 USC
793), FAR 52.222-37 Employment Reports on Disabled Veterans and
Veterans of the Vietnam Era (38 USC 4212) . The provision at FAR
52.212-2, Evaluation-Commercial Items (Jan 99) applies to this
acquisition; the Government will award a contract resulting from this
solicitation to the responsible offeror whose offer conforming to the
solicitation will be most advantageous to the Government based on the
following factors listed in order of importance: 1) Ability of
audio/system to meet minimum government performance requirement; 2)
Contractor_s past performance; 3) Total Cost. The Government will
conduct an assessment of performance risk based on the offeror_s recent
and relevant performance. In assessing performance risk, the Government
will (a) seek questionnaires and (b) usedata independently obtained
from other Government and commercial sources. The purpose of this
assessment is to identify and review relevant present and past
performance and then make an overall risk assessment of the offeror_s
ability to fulfill this requirement. Offerors shall provide a list of
at least two of the most recent and relevant past and present contracts
fulfilled for federal agencies and commercial customers within the last
three years. Furnish the following information for each contract: (a)
company/division name, (b) item(s) provided, (c) contracting agency,
(d) contract number, (e) contract dollar amount, (f) date of delivery,
(g) name, address, and telephone number of the contracting officer,
and (h) comments regarding the contract performance. Offers are due by
