Medical Center, San Francisco, require services provided by a Radiation
Oncologist for Brachytherapy cases. It is estimated 30 to 40
brachytherapy cases in one (1) year. Period of performance is April 1,
2000 (or date of award, whichever is later) through March 31, 2001,
with one, one-year option period. The Brachytherapy procedures will
consist of, but not limited to; (1) Consultation with patient to
reconfirm suitability for brachytherapy treatment; (2) Brachytherapy
dose pre-plan dosimetry; (3) Clinical supervision and participation in
dose implantation in conjunction with VA Urology; and (4)
Brachytherapy post-plan dosimetry, this shall be limited to the
assessment of the post-procedure dose placement and shall not include
performance of any needed CT scans. All participating physicians shall
have active Radiation Oncology clinical privileges at the VA San
Francisco. All participating physicians shall comply with NRC-mandated
Quality Management Form procedures. All other information is provided
