synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. The solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 97-15. This procurement is total small business
set-aside. The standard industrial classification (SIC) code is 7362,
Temporary Help Supply Services. The small business size standard is $5
million average annual receipts for the past three years. Offerors are
encouraged to submit quotes for up to four (4) medical clerks,
full-time, not to exceed 240 working days from date of award. Proposed
rates should include all costs associated with providing the following
services. In accordance with 5 CFR 300, the Lower Michigan Acquisition
Center, Detroit, Michigan, has a requirement for up to four (4) full
time medical clerks for two day shifts, 7:30 a.m. to 4:00 p.m., and two
evening shifts, 3:30 p.m. to midnight, five (5) days per week, for a
period of service not to exceed 240 working days from date of award.
Services will be performed at the VA Ann Arbor Healthcare System, 2215
Fuller Road, Ann Arbor, Michigan. Individuals working under this
contract shall provide clerical support services as a receptionist,
record keeper, and shall be responsible to accurately carry out
physician's orders in requesting examinations, tests, consultations,
including but not limited to scheduling test and examinations required
along with treatments provided by other services. The accurate
performance of these duties not only has an impact on the quality of
patient care, but also the effectiveness and efficiency with which care
can be provided. All staff will start on day shift for training.
Contract staff shall be available to work federal holidays. Work is
performed in or near the nurses' station on an inpatient care unit. The
incumbent works in close proximity to patients and could be exposed to
communicable disease. In the performance of official duties, the
incumbent has regular access to printed and electronic files containing
sensitive data which must be protected under the provision of the
Privacy Act of 1974 and other applicable laws, federal regulation, VA
statutes and policy and DM&S Policy. The contract staff shall be
responsible for: (1) Protecting that data from unauthorized release or
from loss, alteration or unauthorized deletion. (2) Following
applicable computer rules, release of access codes, etc. as set out in
a computer access agreement which the incumbent signs. This position
is that of a medical clerk in Ward Administration Section of Medical
Administration Service. Incumbent is responsible for the integration
and coordination of administrative functions associated in the care and
treatment of patients on Medical, Surgical, Psychiatry, Neurology,
Extended Care and Intensive Care Unit of the Medical Center. The
incumbent may be responsible for coverage on irregular tours, holidays,
and weekends. Major duties are as follows: (1)Interprets and transfers
a variety of doctor's orders for patient examination, consultation,
therapy, tests, diet, x-ray, treatment and activities to a wide variety
of forms, quickly and accurately. Many orders are detailed and complex
and must be able to recognize STAT and emergency orders. Charts are
brought to the attention of the nurse in charge for orders that need
attention, (2) Upon receipt and discharge or transfer of patients,
assembles patient's medical record in correct sequential and
chronological order, (3) Responsible for the compilation, analysis and
accuracy of patient's Consolidated Health Record (CHR), (4) Receives,
refers and files the results of various test in the patients' chart,
keeping the chart current and accurate. Records and relay telephone
messages pertaining to, such things as detailed laboratory reports,
x-rays and test results,(5) Inserts additional forms in the charts as
necessary and imprints with correct patient identification. Services
charts by transferring ethos forms that are approved for removal from
the active chart to the inactive chart, (6) arranges appointments for
the patient's with clinics, services and consultations, (7) Assist in
the orientation of the clinical staff on administrative matters, as
well as VA Regulations and procedures. Maintains a close work
relationship with the clinical staff on assigned wards. Acts as the
resource person to the unit and the clinical staff concerning
administrative procedures and policies, (8) Determines eligibility for
such things as outpatient care, eyeglasses and some prosthetics
appliances and reports data to the physician or nurse planning
treatment, (9) Responsible for arranging patient's follow-up
appointments with the outpatient clinics, (10) Arranges transportation
for patients being transferred to other VA facilities, or (CNH)
Contract Nursing Homes, etc, (11) Responsible for data entry into the
Centralized Patient Records System (CPRS), (12) Maintains bed control
on the Ward Morning Report and makes entries in the computer plus
Record Tracking entries, (13) Receives visitors and telephone calls to
the unit and places official telephone calls. Gives messages to
patients, doctors, nurses and responds to routine inquires.
Appropriately refers questions relating to patient's condition and
maintains confidentiality, (14) Completes clerical process for
admission, discharge, inter-ward transfers, death, authorized absence
and Seriously Ill List, (15) Maintains printer, including adding paper
and changing ribbons. Maintains a supply of computer paper on the
clinical unit. The contract employees will be required to observe all
Medical Center policies and memorandums. The provision at 52.212-1,

(JAN 1999), applies to this procurement. Evaluation criteria including
in paragraph a are, in descending order of importance, (1) Past
Performance and (2) price. Past Performance is more important than
Price. Offerors are required to include a completed copy of the
provision at 52.212.3, Offeror Representations and Certifications --
Commercial Items ALT III (JAN 1999), with its offer. The clause at

applies to this acquisition with the following clauses added as addenda
