for commercial services prepared in accordance with the format in
Subpart 12.6, as supplemented with additional information included in
this notice. This announcement constitutes the only solicitation for
which proposals are being requested and a written solicitation will not
be issued. This solicitation is issued as a Request for Proposal (RFP)
and the solicitation number is DAHA23-00-Q-0019. Solicitation document
and incorporated provision and clauses are current through Federal
Acquisition Circular 97-13. This procurement is solicited on an
unrestricted basis using SIC code 7699. Statement of work requires
picking up modules at Houston AASF, Bldg 1183, Ellington Field,
Houston, Texas, and returning to same location. Contractor will be
responsible for supplying containers for modules and for transportation
to and from Houston, Texas. Contractor is to completely disassemble,
clean, and evaluate for cost to repair 2 each Power Turbine Modules
T700-701 S/N Numbers GE-L-004322 and GE-L-004744 from an AH64A
Helicopter engine, Technical Manual Number TM55-1520-238-23P. If Unit
does not want modules repaired, Contractor shall reassemble the PT
modules and return to Houston, Texas. Time allocated for the tear down,
evaluation, and quotation for repair is 30 days from date of contract
and receipt of modules. Company warrants that they are a "FAA Turbine
Engine Repair Facility". Work shall be performed in accordance with
DMWR 1-2840-248-1/2. No repairs shall be performed on modules until
Agency has been notified of actual cost and has given a "Notice to
Proceed". In submitting repair proposal, Contractor shall provide "Firm
Fixed Repair Cost" to agency. Said proposal shall consist of the
following information: 1. Total labor hours ______ @ $______per hour =
$______ (Prevailing wage rates are applicable); 2. Parts $______
(Submit itemized list of parts and price of parts = $______ (Note:
Agency reserves the right to provide any required repair parts); 3.
Contractor will provide a total cost for repair of each module.
Proposals shall include technical proposal, cost proposal; and past
performance proposal. Submit 3 references of similar work to include
description of work, point(s) of contact, and telephone numbers.
Include a completed copy of the provision at FAR 52.212-3, FAR Clause
52.212-4, Contract Terms and Conditions applies to this acquisition.
FAR Clause 52.212-5 Contract Terms and Conditions Required to Implement
Statutes or Executive Orders -- -Commercial Items apply to this
acquisition. Acquisitions of Commercial Items, 252-225-7001 &
252.225-7036 apply to this acquisition. The following contract clauses
apply to acquisition: FAR 52.222-21, DFARS 252.225-7007, FAR
52.222-26, FAR 52.222-35, FAR 52.222-36, FAR 52.222-37, FAR 52.222.41,
FAR 52.222.42 and FAR 52.232-34. Proposals are due on 17 February 2000
at 1000 hours. Mailed or faxed proposals are acceptable. Mail proposals
