TO THE STATEMENT OF WORK AND THE REQUEST FOR QUOTATION. THE FOLLOWING
INFORMATION IS INCORPORATED INTO THE REQUEST FOR QUOTATION AND THE

Parking Structure and the one (1) Montgomery KONE down escalator at the
main entrance to the Hoover Dam Visitor's Center completed prior to
March 6, 2000 but the latest date the Government will accept for the

of work for the two (2) Montgomery Elevator Inc. traction elevators (E5
and E6) located at the Hoover Dam Parking Structure in Clark County,
Nevada: after completing removal of the failed seals in the Montgomery
traction elevators (E5 and E6), the contractor will remove and inspect
the bearings with a Government representative present, so a
determination may be made as to whether the bearings need to be
replaced prior to reassembly with the new seals. If it is determined
that the bearings need to be replaced, the contractor will provide the
needed parts and labor. The price for the bearing parts and labor
shall be listed in the Request for Quotation as OPTION 1 and listed as
a UNIT PRICE PER ELEVATOR. This unit price per elevator for the new
bearings shall reflect all costs for a complete installation per
elevator including labor, parts, materials, supervision,
transportation, tools, equipment, and disposal of removed parts,
materials, solvents, rags, etc. The new bearings shall be the correct
type for the Montgomery traction elevators (E5 and E6). All removed
materials, solvents, rags, etc. shall be disposed of by the contractor
in accordance with all Federal, state, and local laws, rules, and
regulations. All required disposal documents shall be provided to the
Contracting Officer. All work is to be performed during the regular
working hours of the elevator/escalator trade (7 a.m. to 3:30 p.m.,
Monday through Friday). The Government may exercise OPTION 1 if it is
determined the elevator bearings need to be replaced. Quotes will be
evaluated on the prices for Item 0001, Item 0002, and OPTION 1. FAR
