synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6 as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; quotations are being requested and a written solicitation
will not be issued. The solicitation number RFQ 600-023-00 is being
issued as request for quotation (RFQ) which incorporates provision and
clauses through Federal Acquisition Circular 97-14. This solicitation
is a total set-aside for small business concerns and only quotations
from small business concerns will be considered. The standard
industrial classification code 8099. The anticipated base year
performance from March 10, 2000 to March 9, 2001 with renewal options
for one (1) additional year. Contractor shall quote pricing per on a
hourly rate basis. Schedules/ Price Cost: Line item #1: (Base Period
3/10/2000 to 3/09/2001) Provide on-site ARRT licensed Diagnostic/CT
Technologists service. Estimated 2,300 hrs. Line item # 2: (Option I:
3/10/2001 to 3/09/2002) Provide on-site ARRT licensed Diagnostic CT
Technologists service. Estimated 2,300 hrs. Line item #3: (Base Period
3/10/2000 to 3/09/2001) Provide on-site ARDMS licensed Ultrasound
Technologists service. Estimated 488 hrs. Line item #4: (Base Period
3/10/2001 to 3/09/2002) Provide on-site ARDMS licensed Ultrasound
Technologists service. Estimated 488 hrs. Contract ARRT or ARDMS
Technologist to augment in house staffing shortages during the normal
hours Monday through Friday 8:00 am to 4:30 PM or 10:00 am to 6:30 PM,
excluding legal holiday at the VA Long Beach Healthcare Center, 5901
E. Seventh St. Long Beach, CA. The anticipated requirements for the
contract period will vary depending on staffing and patient level
however the VA will provide the contractor at least one week advance
notice whenever possible. Emergency coverage by Ultrasound Technologist
to be provided on an as needed basis during after hours, weekends and
holidays. Emergencies may require short notice (24) hours. It should be
noted that overtime in excess of 8 hours per day is not expected
however, should a patient procedure continue past the end of shift, the
technologist will be requested to complete the procedure. Overtime rate
of pay in excess of the shift will be one and one/half the basic rate.
CLINICAL DUTIES AND RESPONSIBILITIES: (a) ARRT Diagnostic/CT
Technologists performs routine procedures of the bones, joints, spines
and skull. Must be capable of performing all diagnostic studies. In
addition must be able to perform UGI's Bes, and IVP's in accordance
with the facility protocol. Should also possess the ability to operate
spiral CT. (b) ARDMS Ultrasound Technologists, duties include general
sonography procedures of Abdomen, Liver, Gall Bladder and DVT's.
Perform vascular duplex color doppler evaluations of carotid arteries
and deep venous thrombosis. Must be able to operate Acuson, Hitachi and
ATL equipment. (c) ARRT Technologist and ARDMS Ultrasound Technologist
will be under general supervision of the Chief Radiology Technologist.
(d) Registry shall screen all technologists before making referrals in
order to determine the qualifications and competence of said
personnel. Competency requirements will define by VA Medical Center.
(e). VA reserves the right to examine, approve or disapprove
individuals based on information provided in credentials an/or resumes.
(f) American Registry of Radiologic Technologists and American Registry
of Diagnostic Medical Sonographers shall be licensed in a State,
Territory, or Commonwealth of the United States of the District of
Columbia required to show proof of licensure and carry license with
them all time during the assignment to the VA. (g) The operation of
services contracted for at the Medical Center will be provided by the
VA levels mutually agreed upon which are compatible with the safety of
the patient and personnel and with quality medical care. (h) Contract
personnel shall be responsible for compliance with all procedures in
accordance with applicable VA Medical Center written policies,
procedures and protocols. (i) The successful offeror shall commit to
providing a backup locum ARRT and ARDMS technologists who posses the
same qualifications as those required for the primary technologists.
(j) Persons provided by the Contractor shall be technically proficient
in the skills necessary to fulfill the Government's requirements, to
include the ability to speak, understand, read and write English
fluently. (k) Gather information on patients screened and report
findings. (l) Personnel Policy: The contractor shall be responsible for
protecting the personnel furnishing services under this contract. To
carry out these responsibilities the contractor shall provide the
following: (1) Medical Liability Insurance. ( 2). Worker's
Compensation, (3). General Liability Insurance. (4) Income Tax
Withholding (5) Social Security Payments. The parties agree that such
personnel shall not be considered VA employees for any purpose and
shall be considered employees of the contractor. QUALIFICATION: Only
qualified offers will be considered and evidence of qualifications and
experience must be submitted. 1) Technical Ability: (a) A detailed
plan shall be submitted describing the firm's ability to support the
Technologists. The narrative shall show evidence of reliability,
financial responsibility, and credibility. This plan shall include the
locations of offices, phone numbers; number of staffing, number of
years in business and managerial plan. (b) Agency enforcement of
standards of review and evaluation to ensure ongoing quality service
from health care professionals. (c) Provide contingency plans to be
used in the event of unexpected absences or required replacement due to
rejection of contract personnel. Contractor must maintain a minimum of
three ARRT and ARDMS technologists each category. (d) Provide an
employee's resumes including experience, expertise, education,
background and any unique skills at least three qualified technologists
available from your registry to establish pool. (e) Must possess
minimum of BLS certificate. (f) Diagnostic/CT Technologist must be ARRT
certified, have a minimum of 3 years experience working in a hospital
setting as a Diagnostic Radiology Technologists. Experience in
operation of Computed Radiograph (CR) equipment desired (g) ARDMS
certified Ultrasound Technologist must be specialized in abdominal
organs and have a minimum of 2 years experience working in a hospital
setting as a Ultrasound Technologists. (h) Provide evidence that
technologists are bonded and insured by the agency. 2) Past
Performance- Offeror shall submit a list of (3) current or recently
completed contracts performed within the past year from the date of
this solicitation. As a minimum, contracts listed may include those
entered into with the Federal Government, State or local agencies or
commercial customers. For each contract listed, provide the name of
customer organization; contract number; contract value; and name,
address, and phone and fax numbers of the customer. EVALUATION FACTORS:
52-212-2 Evaluation-Commercial Items: Offeror's experience will be
evaluated as it relates to the services requested. The offeror whose
quotation represents the best value to the Government will be awarded.
Quotations will be evaluated on the following criteria, ranked in
order of importance. Factor I: Technical Ability of firms and
experience of the proposed locum technologists, Factor II: Past
performance; Factor III: Price. All evaluation factors when combined
are slightly more important than price. The following Federal

without discussions. Therefore, each initial quotation should contain
the offeror's best terms and conditions from a price and technical
standpoint. This procurement is subject to availability of funds. All
offers shall be clearly marked with the offerors name and RFQ number.
