65 -- NOTICE OF INTENT TO SOLE SOURCE SOL RFQ:553-44-00 DUE 021800 POC Ms. Barbara Wilkinson, 313-576-3737 The Department of Veterans Affairs, John D. Dingell Medical Center, 4646 John R. Street, Detroit, MI 48201 intends to negotiate with Prosthetic Design, Inc., 305 Smith Drive, Clayton, Ohio 45315, for the purchase of one Pro-Series 11060 Lathe (Large Carver) w/AFO Opiton, includes intatllation, training, PDI representative's expenses, travel and accommodations, plaster or foam blank mandrel, body jacket mandrel, chuck wrench and 1 year warranty. This is a combined synopsis/solicitaiton for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Solicitation number RFJQ 553-44-00 has been assigned to this procurement. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-13, SIC (3842). Vendor shall provide all labor, material, tools and equipment necessary to furnish and install PDI CAM Pro-Series 11060 Lahte. Equipment shall be shipped FOB Destination. The equipment MUST be compatible with the TracerCAD System, Premier Configuration and enable hospital personnel to carve a fully formed AFO. Vendor shall provide training to hospital personnel at a mutually agreed upon time with Prothetics and Company. Cost for same is included in the contract price. This will be a firm fixed price contract and must include all cost associated with the delivery, training and installation of the equipment. Vendor shall provide a full one-year warranty on all parts and labor from final date of final acceptance by the Government. Payments will be made in arrears on certified invoice upon final acceptance by the Government.
