The Contractor shall provide all necessary personnel, supplies, transportation, property and malpractice insurance to provide outpatient, basic diagnostic, preventative and restorative dental services to headstart children at Chinle, Fort Defiance, Gallup, Kayenta, Tuba City, Shiprock and Winslow Service Units. The Contractor shall provide basic diagnostic, restorative and preventative dental services for the purpose of maintaining and improving oral health of Navajo headstart children. Indian Health Service's policies and procedures shall be followed in the performance of this contract. Contractor shall provide dental services to an estimated 1540 headstart children. Anticipated award is for a twelve-month period with two (2) one-year option periods.
