commercial services prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; offers are
being requested and a written solicitation will not be issued.
Solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-15. This procurement
is set-aside for small businesses. The SIC Code is 7363 and the small
business size standard is $5.0 million. This solicitation is being
issued as a Request for Quote (RFQ). All RFQ's are due on February 29,
2000, 2:00 p.m. local time. If mailed submit to Acquisition Service

DESCRIPTION, QUANTITY, UNIT, UNIT PRICE, TOTAL: Bid Item 1: Provide 15
pharmacy technicians which will be required to work Monday thru
Friday, 6:30 a.m. until 3:00 p.m. (first shift) or 3:30 p.m. until
12:00 a.m. (second shift). The 15 pharmacy technicians will be split
between the two shifts as work dictates. Award will be based on an
hourly rate. $________. Contractor shall provide a total of 15 pharmacy
technicians for the Consolidated Mail Outpatient Pharmacy, 5171 Sam
Jared Rd, Murfreesboro, TN 37130, for the period 3-15-00 to 10-16-00.
All positions/shifts include an unpaid 30 minute lunch/dinner break
during work hours. Payment will be made for the actual number of hours
worked as determined by the time sheets kept in CMOP and reported
bi-weekly by the contractor. Payment will be made to the contractor
bi-weekly in arrears. DUTIES: a. Properly operate an automation or
manual area as assigned, utilize specialized equipment and software; b.
Replenish Baker cells according to set policy in an accurate and timely
manner. Clean cells per maintenance program. (Baker cells are part of
the automated tablet/capsule counting system.); c. Manage printers,
bottle hoppers, capper and conveyor systems as assigned; d. Operate the
Dispense-A-Matic automated system as well as replenish stock in an
accurate and timely manner; e. Label and/or fill prescriptions
accurately and per the production standard of the operating system,
including bulk items; f. Participate in inventory, including labeling,
receiving, stickering and rotation of stock, or other inventory
projects (baseline inventory); g. Package mail-outs as per policy and
procedure. KNOWLEDGE/SKILLS REQUIRED(QUALIFICATIONS) a. Knowledge of
and skill in filling prescriptions.; b. Knowledge of brand and generic
names of approximately 6,000 items stocked in the pharmacy; c.
Knowledge of dosage forms and classification of drug to insure proper
labeling and mail/packaging requirements;. d. Ability to read English;
e. Must be 17 years old and a High School Graduate or have GED; f.
Ability to stand long periods, lift, twist, and stoop; g. Manual
dexterity to manipulate totes on and off belt and seal packages of all
sizes; h. Ability to move stock and/or packages from one area to
another; i. No history of drug abuse or criminal record. Personnel MUST
meet one of the following: 1. One (1) year experience in a retail or
hospital pharmacy setting; OR 2. Be a graduate of an Academic
Technician Training Educational Program of a least one (1) year
duration; OR 3. Be "Certified Technician" by a State Board of Pharmacy
or State Pharmacy Association. Qualifications of contractor employees
shall be subject to review by the CMOP Director. The Government has
the right to determine whether or not an individual is suitably
qualified. CONTRACTOR RESPONSIBILITES: The contractor shall ensure each
person performing work under this contract has the appropriate training
and/or education. The Department of Veteran Affairs CMOP is not liable
for injury either personal or property to the contractor or his/her
employees that may occur during the performance of the contract. The
contractor must procure and maintain for the duration of the contract
period, General Liability, Workman's Compensation and Employers Public
Liability Insurance in accordance with the State of Tenn. Before
commencing work under the contract, the contractor shall furnish a
certificate of insurance indicating the coverage. The contractor shall
keep records and withhold appropriate Federal and State taxes and
fees. The contractor shall ensure that all employees receive mandatory
training as required by OSHA, JCAHO. The contractor shall ensure
proper dress and conduct of employees. Uniforms are not required.
Supervision of contractor employees is the responsibility of the
contractor. The contract employees will be required to observe all CMOP
policies and memorandums. The Government reserves the right to remove
any contractor personnel not conforming to CMOP policies. The
professional/legal responsibility for services provided resides with
the CMOP Director or designee, who oversees and checks all services
provided by the contractor. WORK HOURS: a. Regular hours of operation
consist of two shifts, 6:30 AM to 3:00 PM and 3:30 PM to 12 AM, Monday
through Friday. b. It will be necessary to work on some Saturdays and
Federal Holidays. When this occurs advance notice will be given as
early as possible. OFFEROR QUALIFICATIONS: Offers will be considered
only from offerors who are regularly established in the business called
for and who are financially responsible and have the necessary
personnel to furnish services in the volume required for items under
this contract. Successful offeror shall meet all requirements of
Federal, State, County, or City codes regarding operations of this type
of service. ON SITE ADMINISTRATION: Contractor shall provide on-site
personnel to monitor daily attendance and orientation of new staff,
conduct and payroll issues, schedule appropriate number of individuals
as required, and any other performance issues noted by the Contracting
Officer's Technical Representative (COTR). On-site contractor personnel
shall be provided appropriate equipment by the contractor such as
laptop, cellular phone. Contractor shall provide all necessary
equipment (and/or supplies) for timekeeping purposes. This
synopsis/solicitation incorporates one or more clauses by reference,
with the same force and effect as if they were given in full text. The

ADDENDUM TO 52.212-1: Contractor must submit three (3) copies of their
offer. Provision 52.212-2, Evaluation -- Commercial Items, is
applicable. The evaluation criteria is as follows: A. Past Performance:
The contractor must have a minimum of 2 years experience and is
required to furnish the actual number of years in the business of
furnishing Pharmacy Technicians. The offeror must provide 3 references
for past performance. The government may elect to use data obtained
from other sources for evaluation purposes. The burden of providing
current and complete past performance references is the responsibility
of the offeror. The past performance references may be used to
determine the responsibility determination and best value decision. B.
Technical: A detailed plan shall be submitted describing the firm's
ability to furnish 15 Pharmacy Technicians. The narrative shall show
evidence of reliability, financial responsibility and credibility.
Provide contingency plan to be used in the event of unexpected absences
or required replacements due to rejection of contract personnel. Also
include benefit package for employees and plan for supervision. C.
Price: The lowest evaluated price will receive full points. Remaining
proposals will receive points in proportion to the price offered in
comparison to the lowest price. Past performance and technical factors
are listed in descending order of importance and when combined are
equal to price in importance. Offers shall contain sufficient detail in
order to enable government personnel to make a thorough and sound
determination for award. The Government may award a contract on the
basis of initial offers received, without discussions. Each offer
should contain the offeror's best terms from both a technical and price
standpoint. Offerors should submit a completed copies of FAR 52.212-3,
Offeror Representations and Certifications -- Commercial Items and
VAAR 852.219-70 Veteran Owned Small Business with its quotation. Clause
52.212-4, Contract Terms and Conditions -- Commercial Items, applies to
this acquisition and Addendum applies as follows: The following Clauses
are applicable 852.270-1 Representatives of the Contracting Officers;
852.270-4 Commercial Advertising and FAR Clauses 52.224-1 Privacy Act
Notification, 52.224-2 Privacy Act, 52.237-2 Protection of Government

Reference. Clause 52.212-5, Contract Terms and Conditions Required to
Implement Statues or Executive Orders Commercial Items, including
clauses incorporated by reference under section (b) 1, 5, 7, and 11
through 15; (c) 1, 2, and 4. The Government intends to award a
firm-fixed price contract to the offeror that is determined most
advantageous to the Government, price and quality of service
considered. Wage Determination 94-2497 rev. 14, dated 6-1-99,issued by
the Department of Labor states classification 12250 Pharmacy
Technician hourly rate is $11.24, plus health & welfare of $1.63 an
hour, is applicable. Wage Determination dictates: 2 weeks paid vacation
after 1 year of service with a contractor or successor; 3 weeks after
10 years; 4 weeks after 15 years. Length of service includes the whole
span of continuous service with the present contractor or successor,
wherever employed, and with predecessor contractors in the performance
of similar work at the same Federal facility. Holidays: Minimum of ten
paid holidays per year: New Year's Day, Martin Luther King Day,
Washington's Birthday, Memorial Day, Independence Day, Labor Day,
Columbus Day, Veterans' Day, Thanksgiving Day, and Christmas Day. (A
contractor may substitute for any of the named holidays another day off
with pay in accordance with a plan communicated to the employees
