for commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
solicitation number, RFQ 652-90-00 is issued as a Request For
Quotation. The provisions and clauses cited herein are those in effect
through Federal Acquisition Circular 97-15. This is a 100% small
business set-aside. The SIC code is 8042 and the size standard is $5.0
million. The Hunter Holmes McGuire VA Medical Center, 1201 Broad Rock
Blvd., Richmond, VA 23249 is looking for a contractor who can provide
Ophthalmic Technician services on a temporary basis for a period not
to exceed 120 workdays (960 hours). Working hours would be Monday
through Friday, 8:00 a.m. -- 4:30 p.m. The ophthalmic technician would
assist with the direct care of inpatients/outpatients seen in the Eye
Clinic. The volume of this clinic is approximately 6,000 annual visits
per year and is staffed by ophthalmologists and residents from our
affiliated medical school, as well as, from the private sector.
Knowledge required: An understanding of ocular anatomy, physiology and
pharmacology. An understanding of ophthalmic instruments. An
understanding of medical terminology. An ability to assist in minor
office surgical procedures. Demonstrate an ability and willingness to
learn new tasks and concepts. An ability to assist in mobilizing and/or
transferring visually and/or physically disabled patients. Demonstrate
an ability to work with patients and staff in a personable and
professional manner. Must be able to speak English. Principal dutes and
responsibilities: Must be available during Eye Clinic hours (8:00 a.m.
-- 4:30 p.m. Monday through Friday). Assist with general patient
examinations including but not limited to: obtaining screening history
of patient complaints, medical and surgical problem, allegies, current
medications and family history. Obtaining visual acuity with and
without correction. Obtaining spectacle measurement. Refract patients
to obtain initial visual acuity. Perform screening examination to
include ocular motility, pupils, intraocular pressure and screening
slit lamp examination. Understand and perform automated and manual
visual field testing. Perform keratometry on pre and postoperative
patients. Color vision testing. Perform pachymetry. Perform axial
length scanning. Contrast sensitivity testing. Potential acuity meter
testing. Assist and become proficient at spectacle measurement. Assist
and become proficient at spectacle fitting and ordering. Assist in
ordering contact lenses. Assist the ophthalmologist in minor office
surgical procedures. Maintain Eye Clinic equipment inventories and
supplies. Maintain functioning equipment. Ability to recognize and
refer ophthalmic emergencies. Line Item 001. General Assisting
(includes but is not limited to: refractometry, keratometry,
ascans/IOL, calcs, lensometry). Please provide rate per hour; Training
(to include items in General Assisting; Fundus Photos, Fluorescein
Angiography, Goldman Perimetry, Humphrey Perimetry). Please provide
rate per hour. Location of position is in the Eye Clinic at Hunter
Holmes McGuire VA Medical Center, 1201 Broad Rock Blvd., Richmond, VA
