The work to be performed will consist of furnishing all labor, materials, and equipment necessary to construct additions to house a new Medical Records Department and a new Ambulance Carport, the remodel of the existing Records area into two (2) new Pediatric Exam Rooms and Waiting Rooms, and the remodel of the existing Ambulance Garage into two (2) new treatment/Exam Rooms, an new Eye Exam Room, and a new Doctors' Office, and expanison of the existing Telephone Equipment Room. The project will be phased with Government furnished medical filing system equipment installed by the contractor. The plans and specifications use International System of Units (SI) metric equivalent measurements. Project completion time is estimated to be 270 claendar days after issuance of Notice to Proceed. Estimated construction cost is between $250,000.00 and $500,000.00. The proposed contract listed herein is set aside for 51% Indian owned, controlled and operated firms under authority of the Buy Indian Act. Inaccordance with the Public Health Service Acquisition Regulation Paragraph 380.503(e), not more than fifty (50) percent of the work performed under a prime contract awarded pursuant to the Buy Indian Act shall be subcontracted to other than Indian firms. For this purpose, work to be performed does not include the provision of materials, supplies or equipment. A presolicitation notice is being prepared concurrently with this synopsis.
