This is a combined solicitation/synopsis for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number RFQ00Q0038 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14. This action is not a small business set-aside. The SIC code is 3842, and the small business size standard is 500 employees. Offers shall provide a price proposal for the production and packaging of the following item: Item 0001: Chemical Decontamination Kits, Qty 50 each. Contractor shall procure and package the following equipment in each kit: -- Dupont Tyvek-F, Chemical Protective Suit, Stock No. F1414, Qty. 2 Extra Large, 1-Large -- Truetech M295 Chemical Protective Mitt, 8 units per kit -- Butyl Rubber Gloves, .7mm Black, Qty. 2-Extra Large, 1-Large -- Sterile Water, Qty. 1, 500ml bottle -- "V' Blade Safety Rescue Knife, NSN 5110-00-524-6924, Qty. 1 Stebco Knife per kit. -- Terry Cloth Car Wash Mit, Qty 1 Wash Cloth per kit -- Essex Victim Rescue Units, Stock No. MR-10017NSS, Qty. 2 VRU's per kit Bag shall be made of cotton duck fabric. Bag shall be large enough to hold all materials without damage. Bag shall have at least one inch cotton web handles or polyweb handles. Bag shall be black or dark blue in color. Bag shall consist of at least ten ounces of material. Contractor shall submit one (1) complete kit, for first article testing, prior to production. First article shall be submitted within fifteen (15) days from date of award. The Government shall notify the Contractor, in writing, of the conditional approval, approval, disapproval of the first article within 15 calendar days after the Government receives the first article. DELIVERABLES- Chemical Decontamination Kits, Qty. 50, within thirty (30) days after written notification of acceptance of first article. Delivery is FOB Destination with all freight included in the delivered price. Deliver to: US Secret Service, TSD, 843 Brightseat Road (Rear), Landover, MD 20785. The following FAR provisions are applicable to this RFP -- FAR 52.209-4, First Article Approval- Government Testing (Sept 1989) FAR 52.212-1, Instructions to Offerors -- Commercial (June 1999); FAR 52.212-2, Evaluation-Commercial Items (Jan 1999); FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (Oct 1999); FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (May 1999); FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items (May 1999); (additional clauses cited in 52.212-5 that apply to this acquisition are: 1,8,9,11,12,13,14,15,22). Offerors must include a signed copy of the Representations and Certifications with their offers. The contract specialist will not accept telephone call requests for copies of the FAR provisions. EVALUATION PROCEDURES-In accordance with FAR Clause 52.212-2, the following factors are added to paragraph (a): Offerors price and past performance shall be evaluated. Past performance and cost shall be evaluated in accordance with the procedures in FAR Part 15.305. Award shall be made to the offer that is most advantageous to the Government based on price and past performance. The Government intends to award a firm-fixed price contract to the responsible offeror whose proposal, conforming to the items listed herein, will be the most price advantageous to the Government. Award will be on an all-or-none basis with respect to the listed items. All offers, which shall include a include past performance, a price proposal, and signed copy of the Representations and Certifications, shall be mailed to the U.S. Secret Service, Procurement Division, 950 H Street, NW, Suite 6700, Washington, DC 20001 Attn: Renea Morton, for receipt not later than 3:00 p.m. February 25, 2000.
