Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation (F26600-00-Q0052) is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-5. This acquisition is set-aside exclusively for small business concerns. The SIC code for this solicitation is 7699. Far 19 classifies small businesses as those that gross equal to or less than $18.5 million annually based on the average of the offeror_s past three fiscal years. The item description is as follows: Preventive Maintenance Agreement for a RTAS NT Transcription System to include the following: All labor, material equipment, parts to perform 24 hour telephone consultation 365 days a year, hardware and software field engineer change orders and upgrades, intervening repair calls, and all parts and components within the Central Processing Unit Drawer for RTAS NT System and all components with the Central Processing Unit Box. All other terms and condition of this agreement are listed in the statement of work (SOW). The unit is located at Mike O_Callaghan Federal Hospital 4700 N. Las Vegas Blvd N. Las Vegas NV 89191. The period to be covered is from 1 Mar 2000 to 28 Feb 2001 with (4) four option years. There are five line items for this order, one for each year of the contract. Option years to include 1 Mar 2001 _ 28 Feb 2002, and 1 Mar 2002 _ 28 Feb 2003. Award will be based on price. The government will evaluate the total price by adding the total price for the basic 12-month period and four more option periods. The government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the government to exercise the options.
