Department of Veterans Affairs, Central Texas Veterans Health Care System (CTVHCS) Temple, Texas. This is a 100% total set-aside for HUB-Zone Business. (See numbered note 27). The CTVHCS will issue a fixed price requirements contract for Medical Transcription Services. The contractor shall provide medical transcription services to the CTVHCS which includes the VA Medical Centers at Temple, Waco, and Marlin, Texas. Contract period shall be for a period of a base year and three (3) option years. Contractor shall be responsible to provide all necessary labor, supervision, equipment, space, and supplies to perform the contract. The contractor's furnished equipment shall be compatable with equipment already in use by the VA. The furnished equipment shall include as a minimum: Pentium PC with 400/500 MhZ, 64 MB RAM, 6GB EIDE Hard Drive, high speed laser printer. All work stations shall have Windows NT 4.0. The reports shall be simultaneously received in the transcription unit in the PC File and printed. The reports shall be printed to a contractor furnished high-speed printer. The contractor shall maintain patient and records confidentiality at all time. There shall be no exception to this requirement.The contractor shall be in compliance with JCAHO and facility records keeping requirements with an estimated number of lines per workday of 500 with a minimum of 70 characters per line. A complete Statement of Work, Requirements, Specifications, and Evaluation Factors will be included in the solicitation package. All requests for a copy of the solicitation shall be in writing or by FAX. No telephone calls for a solicitation package will be honored. Offers from other than HUB-Zone small business concerns will not be considered. This acquisition shall be conducted in accordance with the provisions of FAR Part 12, Acquisition of Commercial Items.
