(540)-983-1048 This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in FAR Subpart 12.6 as
supplemented with additional information included in this notice. This
procurement is conducted under simplified acquisition procedures in
accordance with FAR Part 13. This announcement constitutes a formal
Request for Proposal (RFP) and a written solicitation WILL NOT be
issued. The solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition 97-15. RFP
658-28-00 should be referenced on all proposals. The Standard
Industrial Classification (SIC) for this procurement is 8099 and the
small business size-standard is gross annual receipts NTE $5.0 million.
This procurement is unrestricted. The VA Medical Center, Salem,
Virginia has an emergent need for a Locum Tenens physician Board
Certified in Oncology to provide oncology/hematology services to
veteran patients. Physician shall provide clinic coverage Monday
through Friday, 8:00 a.m.—4:30 p.m., excluding Federal Holidays.
Lunch is 45 minutes -- 1 hour. The physician shall be assisted by one
full-time nurse practitioner, one Registered Nurse, and one
administrative clerk. The clinic averages approximately 340 patients
per month, including approximately 20 new patient consults. Inpatient
consults equal approximately 8 -- 12 per month. Chemotherapy patients
average 65 -- 85 outpatients per month and 3 -- 6 inpatients per month.
Other duties include 4 -- 8 bone marrow procedures per month;
evaluation of CBC results on patients consulted for hematologic
problems; and review of CT scans, labs, and x-rays of patients seen in
clinic. Physician shall also be responsible for monitoring one
Resident who will see patients with the physician and/or the Nurse
Practitioner. The RN will be responsible for drawing labs. Physician
must be certified in ACLS/BLS. On-call coverage WILL NOT be required.
Physician duties shall be performed within the parameters of the clinic
schedule. Offerors shall submit the following with their proposal:
pricing schedule; curiculum vitae, medical license (from a State,
Territory, or Commonwealth of the United States or the District of
Columbia), evidence of board certification, evidence of ACLS/BLS
Certification and DEA certificate of proposed physician(s); and
evidence of company's Indemnification and Medical Liability Insurance.
Offerors may propose more than one candidate. The Contractor shall be
responsible for protecting the personnel furnishing services under
this contract. To carry out this responsibility, the Contractor shall
provide the following: Worker's Compensation Insurance, Professional
Liability Insurance, Health Examinations (to include current TB Test
results), Income Tax Withholding, and Social Security Payments.
Offerors shall provide a completed set of provisions with their offer
found at FAR 52.212-3, Offeror Representation and
Certifications-Commercial Items. Provisions and clauses found at
52.212-1, 52.212-4, 52.212-5 apply. The following FAR and VAAR clauses
and provisions are added by addendum 52.224-1 Privacy Act
Notification; 52.224-2 Privacy Act; 852.237-7 Indemnification and
Medical Liability Insurance; 852.270-4 Commercial Advertising; and
852.271-70 Services Provided Beneficiaries. Offerors may find the FAR
provisions and clauses on the internet at www.arnet.gov/far/ Proposals
will be accepted immediately, but no later than February 25, 2000. Due
to the critical need of these services, award may be made prior to
February 25, 2000. Offers must be signed by an official authorized to
bind the offeror's organization. All proposals should be submitted to
