modification to the combined synopsis/solicitation for commercial items
prepared in accordance with the format in FAR Subpart 12.6 as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; proposals are being
requested and a written modification to the solicitation will not be
issued. Solicitation RFP 565-15-00 submitted February 11, 2000 is
hereby modified to include the services of an additional locum tenens.
REQUIREMENT: 1 each board certified internist to provide duties and
responsibilities for patients enrolled in Primary Care clinics.
Services will be needed from 8:00 a.m. to 4:30 p.m. Monday through
Friday, excluding Federal holidays for a period effective March 13,
2000 for 60 days with option to extend No payment will be made for the
scheduled half hour lunch break. No on call, weekend coverage or after
hour services will be performed unless specifically stated in awarded
contract. Services will be provided on site at the VA MC, Fayetteville,
N.C. Services will be include provision of appropriate medical
treatment and management of general medical problems of adult patients
in primary care setting. Patients assigned to primary care clinics
will be seen on an outpatient and inpatient basis. Approximately 12
to15 outpatients are seen daily. Conditions requiring specialty care
will be referred to specialty clinics within the VA system. Contract
physician will not refer the patient to outside providers unless
reviewed for eligibility by MAS staff and appropriately authorized. The
contract physician will be expected to prescribe medications for
patients according to the established drug formulary at this facility.
Variance from the formulary should occur only when clinically
indicated and in compliance with procedures established. The contract
physician will be required to enter all reports for incorporation into
patient's record on the day of the visit and will satisfy VHA and
medical center policies and JCAHO requirements for documentation. All
other terms and conditions stated in the original synopsis apply. Cost
Proposal should include an all inclusive rate covering all fees,
professional liability insurance, any housing and transportation costs
involved. Provide hourly rate; overtime rate; holiday rate and on call
availability and rate. Proposals shall be submitted by Noon February
