Gensel (570)-547-6819 (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; quotes are being requested and written solicitation will not be issued. (ii) This solicitation is issued as Request for Quotation (RFQ) 444-0008. (iii) The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular (FAC) 97-14. (iv) This solicitation is open to all temporary service vendors under Standard Industrial Classification (SIC) Code 8051, with a size standard of $5,000,000. Other forms referenced herein are required to be completed and submitted with all quotations. (v) The contractor shall provide unit price for the following: Nursing services per Statement of Work (section "vi"):[1]Base Period (effective date through 60 days)1424 sessions: [2]Option Period 1 (day 61through 120) 1424 sessions: [3]Option Period 2 (day 121 through 240) 2848 sessions. (a chart depicting the price schedule is available via fax) The amounts listed are an estimate for quoting purposes and is not a guarantee of orders or usage. (vi) Statement of Work: Schedule -- The required number of sessions per week is 160. A session is defined as one (1) hour in length. Each nurse will work 40 sessions per week, but not more than 8 sessions per day (See number v above). The above schedule is subject to some variations as the needs of the institution vary. Each nurse may not work more than forty (40) hours per week; Supplies -- All supplies that the contractor may routinely need will be supplied by Health Services Department at Devens FMC; Scope of Service -- Nursing services will include, but are not limited to triage screening of inmates, assisting the physician or mid level provider with procedures, provision of ambulatory nursing care per protocols, evaluations, treatments, and administration of medications as directed by the physician, performing venipunctures, and assisting BOP staff with care of inpatient mental health inmates. Upon completion of the session, the contractor will provide a documented report to the Clinical Director through documentation in the inmate's medical record; Qualifications -- A) Valid and current registration as a professional nurse in a state, District of Columbia, Commonwealth of Puerto Rico, or a territory of the United States. B) Minimum of 3 years recent experience in acute care, ambulatory care, or psychiatric care facility. Experience in correctional nursing preferred, but not required. C) Documentation of medication administration competency. D) Proof of CPR certification. E) ACLS certification preferred; Inmate Management -- Provisions of services under this contract will require frequent and unsupervised contact with inmates. The contractor will not be responsible for the management of inmates other than to ensure that those inmates involved in activities within the contractor's scope of work shall abide by all rules in effect to ensure their safety and well being. The contractor will be required to report/provide documentation to the Contracting Officer's Technical Representative (COTR), or designee, any infractions of institutional rules and regulations by inmates; Institution Security -- The contractor agrees to adhere to all regulations prescribed by the institution for the security, custody and conduct of inmates. The contractor shall attend an orientation program prior to assuming institution involvement. Reimbursement to the contractor for the time spent in institution security/professional related training will be at the contract rate; Other Miscellaneous Information -- In accordance with the Federal Bureau of Prisons Acquisition Policy, Part 37.103(c), the following provisions are applicable to the work being performed in this contract: A) the service is a contractual arrangement and not a personnel appointment, B) payment is based on the provision of an end product or the accomplishment of a specific result, C) the service does not constitute an employer/employee relationship, D) the contractor will not be subject to Government supervision, except for security related matters. However, contractor performance will be closely monitored; Restrictions of Contract Workers -- The Contractor cannot:A) Supervise inmates, B) Supervise Bureau of Prison Staff, C) Participate in inmate accountability, D) Requisition, sign for or pick up supplies, E) Formulate policy or procedures, F) Attend recalls or staff functions, G) Provide escort for visitors; Security Requirements -- The following information is required to be completed before the contractor can receive the contract award in accordance with Bureau of Prisons Acquisition Policy (BPAP) 37.103(b):1) National Crime Information Center (NCIC) check, 2) DOJ-99 (name check), 3)FD-258 (fingerprint check), 4) Law Enforcement Agency checks, 5) Vouchering of Employers over past five years, 6) Resume/Personal Qualifications, 7) OPM-329-A (Authority for Release of Information), 8) NACI check (if applicable) Each prospective contractor is advised that a urinalysis test is required (for detection of marijuana and other drug usage) before the contract is awarded. If the proposed contract is with an individual, and if the test is positive, the proposed contractor shall be disqualified from being awarded the contract. If the proposed contract is with a company and the individual(s) assigned to perform the work test positive, the individual(s) shall be excluded and the proposed contractor shall provide acceptable replacement personnel subject to the same security requirements. (vii) The services are to be performed at Devens FMC Outpatient Clinic and Mental Health Unit, Ayer, MA 01432. The work schedule is defined as follows: (keys: CD = 0730-1600 hrs; CE = 1600 -- 2400 hrs; CM = 0000 -- 0800 hrs) RN-1: Sun(CE),Mon(CE), Thur(CE), Fri(CE), Sat(CE); RN-2: Sun (CM), Wed(CM), Thur(CM), Fri(CM), Sat(CM); RN-3: Sun(CD), Mon(CD), Tue(CD), Wed(CD), Sat(CD); RN-4: Sun(CD), Wed(CD), Thur(CD), Fri(CD), Sat(CM).
