Laurel A. Carey, 301-619-4069 This is a combined synopsis/solicitation
for commercial items prepared in accordance with the format in Federal
Acquisition Regulation (FAR) sub part 12.6., as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation. The Air Force Medical Logistics
Office/Veterans Affairs Special Services contemplates negotiation on a
sole source basis for the CDR System manufactured by Schick
Technologies, Inc. Under the authority of FAR 6.302-1, Only One
Responsible Source and No Other supplies or Services Will Satisfy
Agency Requirements, the Government intends to solicit and negotiate
with only one source for a Firmed-Fixed-Price Indefinite Delivery,
Indefinite Quantity (IDIQ) contract for the basic period of one (1)
year from date of award with four (4) one-year renewal options. The
item was selected by the Air Force after months of
evaluation/field-testing of similar systems. It is approved for patient
care in a field environment for contingency operations and is a
requirement of the standardized Air Force war reserve materiel patient
treatment readiness assemblage. Solicitation number RFQ: has been
assigned to this procurement. Solicitation documents and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-13, SIC 5047 and 3844. Equipment shall be shipped FOB
Destination (within CONUS) within 30 calendar days from receipt of
delivery order. Vendor shall provide a one full year warranty on repair
and or replacement on all components from final date of acceptance by
the Government. The following incorporated provisions and clauses are
those in effect through FAC 97-15 and are applicable to this
solicitation: FAR 52.212-1, Instructions to Offerors Commercial Items:
Addendum 52.212-1, Paragraph (b)(5); Offerors shall provide an express
warranty which at a minimum shall be the same warranty terms, including
offers of extended warranties, offered to the general public; FAR
52.214-4, Contract Terms and Conditions—Commercial Items; FAR
52.212-5, Contract Terms and Conditions Required to Implement Statutes
or Executive Orders -- Commercial Items; Offerors must include (1) a
completed copy of the provision at FAR 52.212-3, Offeror
Representations and Certifications -- Commercial Items, (2) Commercial
and Government Entity (CAGE) code, Contractor Establishment Code (DUNS
number), and Tax Identification number. All FAR provisions and clauses
may be reviewed and/or obtained from the General Service Internet
address http://www.gov/far. This notice of intent is not a request for
competitive proposals. A determination by the Government not to
compete this proposed contract, based upon responses to this notice, is
solely within the discretion of the Government. Information received
will normally be considered solely for the purpose of determining
whether to conduct a competitive procurement. All responses must be
