This solicitation replaces DACW62-99-R-0030 Submission No. 376476 dated September 3, 1999. Solicitation requirements have not changed. Anyone requesting the original solicitation will be furnished a copy of the new solicitation. The Nashville District is soliciting proposals for furnishing of Medical Surveillance Program on a full and open competitive basis, from qualified organizations, to award a contract for one year with two one year options, to be exercised at the Government's request. Medical examinations must be performed by a physician licensed by the State of Tennessee and who meets the examination eligibility requirements of the American Board of Preventive Medicine Inc. Physician assistants are acceptable as long as a licensed physician reviews and co-signs the examination. Basic medical exams will consist of a medical history, occupational history, visual screening, complete urinalysis with emphasis on the following: cardiovascular system, central nervous system, eyes, gastrointestinal system, liver, nasal septum, oral cavity, respiratory system, skin, teeth, and an exam of the employee's back for freedom of motion, etc. Special tests may include pap smear, pelvic exam, breast exam, screening mammogram, hemocult, prostate specific antigen (PSA), CBC/SMAC 21, chest x-ray, EKG, blood lead, cholinesterase-red cell, eye exam, PFT, blood mercury, zinc, protoporphyrin. The contractor will utilize a local laboratory. Examining physician(s) shall be located within a 15 mile radius of the U.S. Federal Building, 801 Broadway, Nashville, TN. Mobile units are not acceptable. Award will not be based on price alone but will be based on the following factors in descending order of importance: Personnel qualifications (including board eligibility by American Board of Preventive Medicine for Occupational Health Physician); ability to perform all examinations/tests at one location; and experience of firm. After evaluation, award will be made as whole to the offeror whose proposal is considered to be the most advantageous to the Government. Award will be made on or about 01 May 2000 for one year with two one (1) year options to be exercised at the Government's request. Standard Industrial Classification is 8011. Size Standard is $5.0 million. Solicitation documents will be available on or about 13 March 2000. Proposals are to be received by the close of business (4:15 Central Standard Time) 13 April 2000.
