Kendrick, Contracting Officer, (540) 983-1048 This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6 as supplemented with additional information contained in this notice. This procurement is being conducted under Simplified Acquisition Procedures in accordance with FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. The solicitation is issued as Request for Proposal (RFP) 658-30-00. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-15. The SIC Code is 8099 with a small business size standard of $5.0 million. The VA Medical Center, Salem, Virginia intends to solicit proposals and award a firm, fixed price contract under the authority of Title 38 USC Section 8153 for a licensed Pharmacist to provide Pharmacist Services 40 hours per week in Pharmacy Service at the VAMC, Salem, VA. The contract is for a six (6) month period but may be extended for an additional six months. Duties shall be as described herein: The Pharmacist shall perform drug dispensing in the outpatient and inpatient sections of Pharmacy, unit dose and IV admixture preparations as assigned. The Pharmacist shall provide pharmacy and medication-related information to patients, VAMC personnel and physicians. Medication orders shall be reviewed, processed and verified for accuracy, drug/food/lab interactions, dosage, directions for use, incompatibilities, polypharmacy, and completeness prior to dispensing. The Pharmacist shall review and verify the accuracy of prefilled prescriptions by Pharmacy Technicians prior to dispensing for patient use. The Pharmacist shall be in compliance with all Rules and Regulations of the VA Medical Center, Salem, Virginia. Hours of performance shall be form 7:45 a.m. to 4:30 p.m., Monday through Friday, excluding Federal Holidays. The ten (10) holidays observed by the Federal Government are: New Year's Day, Martin Luther King, Jr.'s Birthday, Presidents' Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving and Christmas Day and any other day declared by the President of the United States to be a national holiday. Federal Holidays are considered non-work days. The Contractor shall not be required to perform services outside the usual hours of performance. Weekend and on-call coverage is not required. The Pharmacist must have a B.S. or Pharm.D. degree from an accredited school of pharmacy. The Pharmacist must be currently licensed in a State, Territory or Commonwealth of the United States or District of Columbia and be a U.S. citizen. The Contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the Contractor shall provide the following for their personnel: worker's compensation, professional liability insurance, health examinations, income tax withholding and social security payments. The parties agree that such personnel shall not be considered a VAemployee for any purpose and shall be considered employees of the contractor. A record keeping system will be developed by the Pharmacy Manager or his designee of Contractor hours worked. The Contractor shall be paid only for those hours worked, any travel time will not be reimbursed. Payment for any leave, including sick leave, holiday or vacation time is the responsibility of the contractor. Offerors responding to this announcement shall submit a cost proposal to include an all-inclusive hourly rate. Technical proposals shall include the following evaluation factors: Company experience, personnel qualifications to include current licensure, CV, and pharmacy degree; past performance on similar contracts in size and scope over the past three years; and references (3) for the proposed pharmacist. Offerors are advised that if the technical proposals are essentially equal, award will be made on the basis of lowest price. A completed copy of provision 52.232-3, Offeror Representations and Certifications-Commercial Items and 852.219-70, Veteran-Owned Small Business must be submitted with the offer. Proposals including price and technical must be received by Wednesday, March 15, 2000 at 4:30 p.m. (local time).
