The Navajo Area Indian Health Service (NAIHS) intends to solicit the services of Mobile On-Site Mammography Services (MOM), on a sole-source basis, to provide on-site mammography diagnostic and screening services for NAIHS medical facilities as follows: Tuba City Indian Medical Center, Tuba City, Arizona; Kayenta Health Center, Kayenta, Arizona; Fort Defiance Indian Hospital, Fort Defiance, Arizona; and Chinle Comprehensive Health Care Facility, Chinle, Arizona. This requirement meets the provision for sole source procurement cited under FAR 6.302-1 (2). The procurement requires the Contractor to possess a current State of Arizona certified mammography contract with the Navajo Nation to allow for coordination and billing of alternate resources applicable to mammography services. The Contractor shall provide all necessary personnel, travel, per diem, medical equipment & supplies, laboratory services, medical liability insurance, etc. The Contractor shall also provide interpretation and transcriptionassociated with the mammography studies. The actual site visit dates shall be coordinated with each Service Unit Site Coordinator. The Contractor's staff must be certified/licensed by the appropriate State or Medical Boards.
