The contractor shall furnish all personnel, labor, uniforms, training, materials, equipment, supervision and other items of services required to manage and provide ground emergency ambulance services, emergency room support functions, and rountine patient transportation services on a 24-hour per day basis, as required for the service area of Bayne Jones Army Community Hospital (BJACH), an Army Medical Treatment Facility (MTF) located at Fort Polk, LA. This service shall be provided in accordance with Louisiana State Laws, Federal Laws, Army Regulations and the terms of this contract. A current personnel roster and updates of changes to the personnel roster shall be delivered to the Contracting Officer's Representative (COR) a minimum of 24 hours prior to the individuals working under the terms of this contract. Roster shall list name, title of each individual, level of Emergency Medical Technician (EMT) proficiency, certification numbers and expiration dates for licenses and certificates.
