A solicitation will be issued. Request For Quotation (RFQ 623-12-00) is assigned and shall be referenced on any Quotations. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14. The Standard Industrial Classification Code is 8331 and the small business size standard is $5 million. The VA Medical Center, located at 1011 Honor Heights, Oklahoma 74401has a requirement for vocational rehabilitative services, specifically; case management services. Contractor shall provide case management for persons undergoing vocational rehabilitation. Services provided include but are not limited to: face-to-face counseling, telephone counseling, verification of status, validation of curriculum, comprehensive reporting, and referral for employment services. The government anticipates award of multiple Blanket Purchase Agreements for these services.
