A solicitation will be issued. Request For Quotation (RFQ) 623-11-00 is assigned and shall be referenced on any Quotations. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14. The Standard Industrial Classification Code is 8331 and the small business size standard is $5 million. The VA Medical Center, located at 1011 Honor Heights, Oklahoma 74401has a requirement for vocational rehabilitative services, specifically; counseling services. Contractor shall provide Chapter 31 Initial Evaluations and Appraisals, including Employability Assessments. Purpose of the service is to gather entitlement information, which is to be used in developing rehabilitation plans and assist our clients with decisions, concerning education and careers. The process includes counseling interviews, tests that assess interests, aptitudes and abilities as appropriate to the individual counselee and information as it relates to the world of work or education. The government anticipates award of multiple Blanket Purchase Agreements for these services.
