This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6 and 13.5 as supplemented, with the additional information included in this notice. This announcement constitutes the only solicitation; This is a Request for Quote and a written solicitation will not be issued. Solicitation number RFQ 667-15-00; award will be made using the simplified acquisition procedures. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-15. This acquisition is unrestricted. The standard industrial classification (SIC) 7361. REQUIREMENT: Contractor to provide one (1) Dermatologist, all inclusive rate, to perform services, approximately 40 hours per week for approximately three to four months. Contractor shall provide all liability insurance, services, lodging, transportation, communications and associated expenses for the ontract period. Individual performing under this contract shallprovide the ecessary documentation for rivileging purposes at this VAMC. Services shall commence immediately after award but not later than March 20, 2000. The Dermatologist will be required to see, diagnose and manage approximately 30- 40 patients per day. Procedures and services include but are not limited to: Excision of skin lesions, incision and drainage of skin lesions, biopsies and interpret pathology results. Assures content and correctness of clinic visit notes and signs documents to validate their content. Provide rofessional direction and teaching to Dermatology resident physicians in diagnosing and treating disease of skin. QUALIFICATIONS: Personnel under this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia and Board Certified. The qualifications of such personnel shall also be subject to review by the VAMC Chief of Staff and approval by the VAMC Director. Personnel assigned under this contract shall be Board Certified or Board eligible. Board eligible candidates must not exceed 4 years from the date they became eligible. TERM OF THE CONTRACT: Services shall be provided at the Overton Brooks VAMC, 510 East Stoner Avenue, Shreveport, LA 71101, Monday through Friday from 7:30 am to 4:00 pm, except for federal holidays. National Holidays: The ten (10) holidays observed by the Federal Government, i.e., New Years Day, President's Day, Martin Luther King Jr.'s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. The contract resulting from this solicitation shall be for a period of 3 to 4 months with the option to extend for up to 6 additional one month periods. Additionally we have the option to Terminate sooner in the event permanent recruitment efforts prove successful. PAYMENTS: Sums due for actual work done, the Contractor will be paid monthly, in arrears, upon receipt of a properly prepared invoice FAR 52.232-25. The parties agree that the contractor shall not be considered a VAMC employee for any purpose. CONTRACT PERFORMANCE: a. The contractor will be monitored through a variety of mechanisms, including but not limited to: -- Clinic appointment lists showing patients seen. -- sign-in/out log. -- Chart documentation of patient diagnosis, treatment and or follow-up procedure. -- QA monitors designed to assess the quality of care provided in the clinic. b. The Chief of Medical Service (111) will be responsible for monitoring the quality of services provided under this contract. Estimated Hours 680 _________ .
