Veterans Affairs Medical Center, San Francisco, CA., intends to award
a contract for grounds maintenance services for two (2) cemeteries that
require: tree and tree stump removal, pruning, trimming, cabling,
cavity repair, root pruning, removal and disposal of resulting waste
and debris. These services shall be performed at the Golden Gate
National Cemetery, 1300 Sneath Lane, San Bruno, CA. 94066 (San Mateo
County) and the San Francisco National Cemetery, Presidio of San
Francisco, San Francisco, CA 94129 (San Francisco County) for a
one-time basis. The Contractor shall provide all supervision,
professional advice/guidance, labor, parts, materials, equipment, and
personnel to provide these services. THESE SERVICES SHALL BE PERFORMED
UNDER THE INSTRUCTION AND GUIDANCE OF A CONTRACTOR PROVIDED
INTERNATIONAL SOCIETY OF AGRICULTURE "CERTIFIED ARBORIST" The
Contractor shall complete these services within sixty (60) working
days, approximately three months, for both sites combined, from start
to finish.At the Golden Gate National Cemetery there are fifty-seven
(57) trees that require removal and seventy-four (74) stumps that
require grinding in accordance with the specifications. At the San
Francisco National Cemetery there are fifty-nine (59) trees that
require removal and sixty-nine (69) stumps that require grinding in
accordance with the specifications. All equipment and supplies
maintained and operated by the Contractor shall be consistent and fully
compliant with all applicable Federal, State, County, and City laws,
ordiances and regulations; and meet the State of California inspection,
safety, licensing, registration, and insurance requirements. Contractor
must carry worker's compensation and employees liability insurance
(minimum $500,000 per occurrence) in compliance with Sate Laws and, in
addition, must carry liability insurance to cover any Government
property damages or personnel injury resulitng from work under this
contract. Copies of all licenses, permits, insurance documentation;
three (3) commercial and three (3) government references; five (5)
business credit references; and three (3) bank references must be
submitted with proposal for verification. See Note 1. ATTENTION: All
