Contractor shall provide a Physician's Assistant for up to two follow-up appointments per day, as needed, on a fee for service basis (based upon the HCFA fee schedule). The services shall also include the following laboratory resources to accomplish: Hemoglobin A1C (diabetes); Microalbumin (diabetics); and Lipid Profile. The contractor shall provide health screening and education (Prevention Indicators) following appropriate training by VA staff. Provide immunizations using VA-provided vaccines. The location of this vesting clinic must be in the Fredericksburg/Stafford, Virgnia area. This is for the establishment of a temporary Vesting Clinic to serve patients in the Fredericksburg and surrounding counties for a period of approximately six months.
