J -- MAINTENANCE SERVICE CONTRACT ON TWO BIAD DUAL HAED GAMMA CAMERA SYSTEM SOL N00168-00-Q-X201 DUE 031600 POC Ms. Tracy Holland (301) 295-0286 The National Naval Medical Center, in Bethesda, MD intends to award a sole sourced contract to Medical Data Information Service, Inc., for maintenance and service of two (2) Biad dual head Gamma Camera Systems. The maintenance coverage will be performed during the normal business hours of 8:00am through 4:30pm, Monday thru Friday. The contractor shall telephonically respond within four (4) business hours after receipt of equipment failure. The contractor shall provide on-site service within eight(8)hours. Equipment shall be operational within sixteen (16) business hours. All repair parts shall be furnished by the contractor at no cost to the Government. The contractor shall perform two (2) preventive maintenance inspections to be performed within the contract period of 01 Apr 00 through 30 Sept 2000. The preventive maintenance inspections are requested in May 2000 and August 2000. Award company must be actively registered in DoD Central Contractor Registration (CCR), have federal tax identification number, have CAGE code, and DUNS number. NO TELEPHONE CALL WILL BE ACCEPTED.
