synopsis/solicitation for commercial items prepared in accordance with
the format in the Federal Acquisition Regulation (FAR), Subpart 12.6,
as supplemented with additional information included in the notice.
This announcement constitutes the only solicitation; no other document
will be issued. (II) This solicitation is issued as Request for
Quotation (RFQ) 132-9314. (III) This solicitation document and
incorporated provisions are those in effect through Federal Acquisition
Circular (FASC) 97-17. (IV) The Standard Industrial Classification
(SIC) code is 5099 (NAICS code, 42199). (V) In accordance with FAR
Subpart 1104(b), the U.S. Penitentiary, Leavenworth KS, is seeking a
source for a Garaventa Stair-Trac, OR EQUAL. Specifications: Allows an
attendant to transport a person in a wheelchair up and/or down stairs,
in a stairclimbing method; attaches to wheelchair; power to carry
wheelchair approximately thirty (30) floors; load capacity, 285 pounds;
power capacity, 650 steps; motor, 12v DC; batteries, 2x6v -- 20AH;
maximum charge rate, 4 amps; optimum stair angle, 35-degrees; indoor
and/or outdoor capability; detachable support handle; quick-release
safety belt; rechargeable battery and charger. Any offer submitted for
a product other than the stated device must be accompanied by
descriptive literature for comparison purposes. (VI) Delivery to be
made no later than 30 days from date of award. Delivery address: U.S.
Penitentiary, Attn: Receiving Depot, 1300 Metropolitan Avenue,
Leavenworth KS 66048. (VII) FAR Provision 52.212-1, Instructions to
Offerors-Commercial Items, is hereby incuded by reference. The
following addenda are included by reference: At paragraph C, Period of
Acceptance of Offers, vendor agrees to hold the prices offered for
sixty (60) calendar days from the date specified for receipt of offers.
At paragrpah H, Multiple Awards, the Contracting Officer has determined
that multiple awards are not applicable in this solicitation, and the
paragraph is hereby removed in its entirety for the purposes of this
requirement. FAR clause 52.211-6, Brand Name or Equal, is included.
(VIII) FAR provision 52.212-3, Offeror Representations and
Certifications-Commercial Items, will be completed and submitted prior
to award. (IX) Invoices will be submitted to U.S. Penitentiary, Attn:
Accounting Supervisor, 1300 Metropolitan Avenue, Leavenworth KS 66048.
(X) FAR provision 52.232-33, Mandatory Information for Electronic Funds
Transfer Payment, applies to this solicitation. (XI) Offers may NOT be
faxed. Offers and descriptive literature must be mailed to U.S.
Penitentiary, Attn: Supervisory Contract Specialist, 1300 Metropolitan
Avenue, Leavenworth KS 66048. They must be received no later than
3:00pm, April 10, 2000. All offers MUST be clearly marked with the RFQ
