Seefeldt (650) 849-0384 VA Palo Alto Health Care System is soliciting for an organization with the capability of performing the design and execution of health surveys by mail and telephone. Contract involves follow-up of a group of 900 patients with diabetes who receive health services from VA healthcare facilities in California, Wisconsin and Michigan. Contractor will be required to obtain survey information, in English and Spanish, about patients' health and health care use at the time of enrollment (baseline), 3-months, 6-months, 9-months and 12-months. Surveys at baseline and 12 months will be conducted by telephone. Surveys at 3, 6, and 9-months will be done by mail. Completed surveys are to be optically scanned or double-key-entered into an electronic file and converted into a SAS database. Work also includes providing VA Center for Health Care Evaluation (CHCE) with frequency distributions and summary descriptive statistics of the survey responses. Work involves conducting surveys of mobile, low-income individuals, whomay be difficult to locate. Experience doing survey research in VA population is highly preferred. All aspects of the work are to be completed within a three year period, at which time the original surveys and a data file will be transferred to VA CHCE. Contractor will be required to do necessary database programming in order to allow for computer assisted telephone interview (CATI) administration of the telephone surveys, process and print mailed surveys in a format suitable for optical scanning or keypunch entry, develop letters, reminder postcards, envelopes, etc. Weekly communication with VA CHCE will be a requirement.
