Bonner, 440.838.6091 Under the authority of 38 U.S.C. 8153 Expanded Sharing Authority and in accordance with FAR, Part 12 Acquisition of Commercial Items and FAR, Part 15, the Louis Stokes Cleveland, Veterans Affairs Medical Center, 10701 East Boulevard, Cleveland, Ohio, intends to procure services of 1.0 Full Time Equivalent Employee (FTEE) Nuclear Medicine Physician to provide clinical services. The contract physician shall perform studies, provide consultation and advise the referring physician on the imaging procedures and radiopharmaceutical procedures. The contract physician shall be on site at the Cleveland, VAMC, eight hours per day, between the hours of 8:00 a.m., to 4:30 p.m., Monday through Friday, excluding federal holidays. In addition, contractor shall provide on-call service, 24 hours per day, seven days per week, including federal holidays, for consultation by telephone, for a base period from date of award through September 30, 2000 and two, one pre-priced option periods. This acquisition is unrestricted, the Standard Industrial Code is 8062 and the Small Business Size Standard is $5.0 million. The following technical evaluation factors, listed in descending order of importance, shall be used to evaluate all proposals: 1. Technical: I Adherence to service schedule comprised of (a) on-site coverage (b) emergency coverage; i. On-site emergency coverage ii. Teleradiology emergency coverage iii. Combination on-site and teleradiology emergency coverage. II. Experience in Nuclear Medicine Program Management. III. Past Performance on Similar contracts. IV. Experience of Proposed Physician(s) in an academic setting. 2. Price Factors: 1. Price and price related factors.
