This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a (RFQ) DAHA12 00 T 0009. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14. This solicitation is 100% set aside for small business with the exception of proposals that are quoted from a GSA Schedule. The SIC 5416-90 Safety Consulting Services. GENERAL REQUIREMENTS AND RESPONSIBILITIES ARE IDENTIFIED IN THE STATEMENT OF WORK AS FOLLOWS: 1.0 SCOPE 1.1 INTRODUCTION: This Statement of Work (SOW) establishes requirements and responsibilities for the contractor in providing support to initiate ANG cultural surveys, associated training and formal reporting to the Director, Air National Guard (NGB/CF). 1.2 BACKGROUND: The Air National Guard (ANG) is an organization dedicated to mission accomplishment, quality of life and organizational excellence. In the past, the ANG has conducted numerous cultural surveys to determine the strength and deficiencies of an organization's leadership, infrastructure, and ultimately, its ability to succeed with the respective mission These insightful, honest results have been provided to the respective unit Commanders and used to improve organizational processes and procedures, ultimately increasing mission efficiency and effectiveness. 1.3 PURPOSE: 1.3.1 General: The Air National Guard has a requirement for support in conducting a cultural survey throughout its 54 states and territories. "Culture" is defined as the informal working environment of an organization Culture surveys access an organization's level of trust, integrity, and effective communication. The survey process provides field commanders with qualitative and quantitative feedback on their "Unit Culture". A survey of field commanders showed an overwhelming desire to continue the culture survey process These surveys shall include an intense training period for respective units/Guardsmen end will result in reports/briefings to many high level ANG personnel, organizations and training courses. These cultural results shall be integrated into a distinct teaching block for the ANG Senior Commanders Course and Squadron Commanders Course, among others. The detailed requirements for the contractor to support this endeavor will be detailed in the following Statement of Work (SOW). 2.0 REQUIRED DOCUMENTS 2.1 The following regulations and instructions are to be used by the contractor to provide guidance and reference information in the accomplishment of the required tasks in this Statement of Work (SOW): AFPD 91-2 Safety Programs, AFI 91-201 The US Air Force Mishap Prevention Program AFI 91-204 Safety Investigations and Reports, AFJI 91-206 Participation in a Military or Civil Aircraft Accident Safety Investigation, AFI 91-213 Operational Risk Management Program AFPAM 91-215 Operation Risk Management Guidelines and Tools, AFP 127-1 US Air Force Guide to Mishap Investigation AFT 11-2V1 Aircrew Training by IS, AFI 11~2V2 Aircrew Evaluation Criteria by MD AFT 11-2V3 Aircrew Operational Procedures by MDS, AFI 11-209 Air Force Participation in Aerial Events AFMAN 36~2236 Guidebook for Air Force Instructors, 2.2. OTHER GOVERNMENT DOCUMENTS: the contractor to provide guidance and reference information to accomplish the required tasks in this SOW shall use the following: "Culture Workshop" Power Point Briefing located at: Safetycenter.navy.mil/aviation/presentations/presentations.htm. 3.0. REQUIREMENTS The primary contractor personnel performing this effort shall have an understanding of DOD and Air Force mishap investigation procedures, a safety investigation background upon which to apply experience to survey impact, and a general area of expertise required to adequately provide support to include: General survey collection methodology, configuration management, training/instruction, media presentation and report/briefing production. The contractor shall perform the following specific tasks defining the work to be performed. This includes providing analyses reports and survey briefing material/courseware for survey team training. 3.1 REQUIREMENT DEFINITION: The contractor shall provide services and documentation of the follow requirements: 3.1.l In development of the "Cultural Survey" program, the contractor shall coordinate with personnel at the Headquarters, Air Force Safety Center and ANG Chief of Safety to assess Safety Center culture assessments tools and incorporate the tools into the ANG Culture Survey program. 3.1.2 Train four (4), three-person "ANG Culture Survey Teams" resulting in the ability of team members to independently conduct ANG culture surveys for ANG organizations. 3.1.2.1 Coordinate with ANG/DOS staff to solicit volunteer members for the "Cultural Survey Teams" from within the AF and ANG. There will be at least two Fighters, one Tanker, and one Airlift culture survey team. 3.1.2.2 Provide ANG/DOS with a monthly status report on team training status and site visit results in contractor-determined format. 3.1.2 3 Develop and conduct a one-week (including travel) initial training course for at least four (4) three- person Survey Teams. Provide training material (media and print copies) as a contract deliverable to ANG/DOS at contract completion. 3.1.2.4 Accompany Survey Teams on field visits to provide hands-on guidance and assistance to the survey team members (in training) during ANG unit visits. Estimated number of (3) day accompanied visits are 13. Site visits should be considered (5) days including travel. 3.1.2.5 Provide ANG/DOS with monthly status reports on team training status and site visit results in contractor-determined format. 3.1.2.6 Provide ANG/DOS with validation of fully trained survey teams in contractor-determined format. 3.1.2 When tasked by the Director, Air National Guard, contractor shall conduct Culture Survey unit visits. 3.1.2.1 When tasked by Director, Air National Guard, contractor shall attend conferences and present issues relating to ANG Unit Culture and safety. 3.1 .2.2 Contractor may be required to brief MAJCOM commanders and Chief of other services on Unit Culture and its applications. 4. 0 PERIOD OF PERFORMANCE: 1 Apr 00 through 30 Sep 00 (with a one-year option to be exercised upon the Government's request) 5.0 POINT OF CONTACT: ANG point of contact for this effort is Elizabeth M. Sheppe, Maj. Don Phillips. 6.0 TRAVEL basis. Travel will be reimbursed in accordance with DOD Joint Travel Regulation (JTR) Chapter 6, and Federal Acquisition Regulation (FAR) @ 31.205-46. All travel orders will be issued through ANG/DOS. 6.1 Estimated travel the contractor will be required to perform in accomplishing the tasks defined in the SOW will be approximately 20 trips. These trips shall allow the contractor to visit specific unit/organization to conduct cultural surveys, survey team training classes, and associated culture out-briefs. Locations are to be determined (TBD) at a later date but may include ANG organizations throughout the 54 States and Territories under the purview of the Air National Guard. Trip estimates and duration are TRIPS NUMBER DURATION: (1). -Survey Team Recruitment and Screening, 3-5 Day trips. (2). -Survey Team Initial Training, 1-3-Day trip. (3). -Survey Team Accompanied Visits, 13-3 Day trips. (4) -NGB/CF Directed Travel, 3-3 Day trips. 7.0 GOVERNMENT FURNISHED FACILITIES: 7.1 Contractor shall be allowed access to all Air National Guard facilities for purposes of accompanying Survey Teams and conducting NGB/CF directed visits. 7.1.2 Use of DSN phones shall be afforded to the contractor for coordination of site visits when acting on the request of Director of the Air National Guard. Contractor shall furnish all other necessary equipment. Provision at 52.212. -1 Instruction to Offerors-Commercial applies to this acquisition with no addenda. The Provisions at 52.212-2 Evaluation-Commercial, applies to this acquisition. The following Factors shall be used to complete paragraph (a) technical capability and past performance, when combined are greater than price. Offerors shall include a completed copy of the provision at 52.212-3 Offeror Representations and Certifications-Commercial Items with their offer. The Clause 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition with no addenda.
