The VA Medical Center Nashville, TN is seeking offers from medical care providers for Primary Helath Care Services for enrolled veterans. Payment for services will be on a monthly capitated rate basis. The contract term will be one base year with two 1 year options for contract extension. The primary health care services will be provided in Sumner County Tennessee area.Contracted is expected to meet or exceed JCAHO requirements for Primary Care. Facilities must be in compliance with the American Disability Act requirements. The VA is expecting to enroll approximately 500 patients within one year after award. The contractor shall provide all personnel, clinic facilities, equipment, supplies, and supervision for the provision of primary health care services. The contractor will be responsible for the provision of all radiological and laboratory services associated with primary care services. The contractor must have a primary care provider on call 24 hours a day, 365 days a year to respond to enrolled patient telephone inquires. Contract award will be based upon evaluation of offers using selection factors including cost as one factor. Solicitatin documents are expected to be available on or about 3/29/00.
