To purchase Per Diem Home Hospice care services for eligible enrolled veterans with the (VAMC) -- Atlanta. A negotiated Per Diem rate is anticipated based on the current Medicare rates available for this service. The VAMC anticipates multiple contract to cover all the counties we service and to provide veterans with a choice. All interested parties must be certified and licensed as a Hospice by the state and be certified as a Medical Hospice provider by HCFA; and fully accredited by the Joint Commission on Accreditation of Healthcare Organization (JCAHO). The emphasis of this Home Hospice care program is on keeping the hospice patient at home with family and friend as long as possible. As such, interested parties must demonstrate ability to provide such service and ability to assess the medical, physical, social, emotional and spiritual needs of the patient and continue this approach while caring for the patient and family under a developed plan of care. This plan must include asessment of the patients needs, identification of services to be provided including the management of pain and other uncomfortable symptoms and state in detail the scope and frequency of services needed to meet patient's needs. The Hospice Agency will be responsible for establishing and integrating services that ensure the plan of care continues to be reviewed and updated to serve the palliation of the patient's terminal illness. The following services related to the management of terminal illness will be required: Nursing services; Medication (including intravenous fluids); necessary lab testing; patient and family education/counseling on death and dying issues; delivery of medication and supplies; and delivery, assembly, and set-up of equipment in patient's residence.
