a combined synopsis/solicitation for commercial items prepared in
accordance with the format in Subpart 12.6; as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. This solicitation is being
issued as a Request for Quotation (RFQ). This procurement is being
conducted under the Simplified Acquisition Procedures (FAR Part 13).
The solicitation reference number is RFQ 670-27-00 and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular (FAC) 97-15, dated February 25, 2000. This solicitation is
unrestricted; the associated Standard Industrial Classification (SIC)
code is 5047 (Size: 100 employees). The VA Healthcare Network Upstate
NY (VISN 2) intends to purchase twenty -one (21) model 6100
CADD-Prizm VIP system #21-8820 and thirty-seven (37) model 6100
CADD-Prizm PCS system #21-8810 from sims Deltec, 1265 Grey Fox
Road, St. Paul, MN 55112. To be technically acceptable, an equal
product must have the following minimum salient characteristics: Pump
must be FDA approved for Subarachnoid/intrathecal space infusion, the
weight of the pump, empty medicine cassette, and battery must not
exceed 20 ounces, the pump must have the ability to download
information to a PC without additional software, and the BIOMED toolbox
must be fully accessible to Biomed Technicians or authorized personnel
with separate authorization access codes. Interested parties shall
submit information identifying clear and convincing documentation
(including pricing) that they can meet the salient characteristics. No
solicitation document is available. The provisions at FAR 52.212-1

Evaluation-Commercial Items, apply to this acquisition. The evaluation
will be based on Technical acceptability and price. Offerors shall
include a completed copy of the provision at FAR 52.212-3, Offeror
Representations and Certifications- Commercial Items. The clauses at

52.212-5, Contract Terms and Conditions Required to Implement statutes
