This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is being issued as a Request for Quote (RFQ). The solicitation number is RFQ #565-17-00. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14. This is a Total Small Business Set-Aside. The SIC code is 8072 and the small business size standard is $5.0M The VA Medical Center, Fayetteville, N.C. intends to solicit quotes and award a firm fixed price contract on a competitive basis to the offeror(s) whose quotation meets the requirements of this solicitation. Period of contract will be from date of award through September 30, 2000 with two one-year options to extend. Requirements: Provide the following services: A.Crown & Bridge Procedures: Includes all procedures from pour up impressions to finished product. Full Gold Crown/Porcelain-fused gold crown units shall be a minimum of 2% gold metal. Fixed partial Denture Abutments and Pontics (Porcelain fused to Gold) shall be a minimum of 2% gold metal. Price shall include the cost of the gold-based metal. Dies shall be returned for trimming by the dentist. There may be rare instances (once or twice per 12-month period) whereby an additional procedure may be required. Price shall be negotiated between the VA and the Contractor. B.Pick-Up and Delivery: Contractor shall pick up and deliver impressions, teeth, prescriptions, finished product, etc. from and to Dental Service, VA Medical Center, Fayetteville, NC on a daily basis, Monday through Friday, excluding Federal Holidays. Pick-up and delivery shall be between the hours of 8:00 a.m. -- 4:00 p.m. All crown and bridge procedures shall be completed within ten (10) working days from the day of pick-up. All complete and partial denture steps shall be completed within four (4) working days of pick-up. Contractor Requirements: Quotes will only be accepted from those firms that offer complete fixed prosthetic services to include crowns, bridges, pontics, and who can offer daily pick ups and deliveries. Documentation: Offeror must submit a copy of their business license with their quote. Offeror must submit, with their quote, documentation of the type of metal used in furnishing dental products to the VA Medical Center, Fayetteville, NC. Warranty: Contractor will be responsible for remaking all dental products that are not correct due to Contractor's error. If a crown does not fit the die, it will be considered the fault of the Contractor and the crown remade at no additional expense to the Government. If the crown fits the die, but not the patient's mouth, it will be considered the fault of the Government, and the remake will be at the Government's expense. If, in the case of porcelain work, the porcelain shatters within twelve months of fabrication, it will be considered the fault of the Contractor and remade at no additional expense to the Government. Work Hours: Work shall be accomplished Monday through Friday, 8:00 a.m. to 4:00 p.m. National Holidays -- The following ten (10) holidays are observed by the Federal Government as well as any other day specifically declared by the President of the United States of America to be a national holiday: New Year's Day, Labor Day, Martin Luther King's, Jr.'s Birthday, Columbus Day, Presidents' Day, Veterans Day, Memorial Day, Thanksgiving Day, Independence Day, Christmas Day.
