York VA Medical Center, Murfreesboro, Tennessee, has a requirement for a Community Based Outpatient Clinic (CBOC) for the Cookeville, Tennessee area. The contractor shall provide for the delivery and management of continuous primary and preventive medical care for enrolled VA patients, to include health care providers, medical facility, equipment, supplies, lab, radiology, short term pharmaceuticals and all administrative functions in accordance with terms and conditions of the contract. Medical care providers operating under the resulting contract must meet all evaluation criteria and other professional certifications required to operate in such a clinic. Compensation will be made through a monthly capitation per patient, per month, payment. Payment made by the VA will be all inclusive. The contract expected to be awarded from this solicitation will include a base year and two (2) one (1) year renewal options which may be exercised at the option of the Government. Award pending availability of funds. This is an RFP, contractor shall address the evaluation criteria listed in the solicitation document. Services will be procured in accordance with Federal Acquisition Regulation Part 12, Acquisition of Commercial Items. A preproposal meeting will be held, specific date and time to be determined.
