The VA Medical & Regional Office Center, 215 North Main St., White River Junction, VT 05009, has a requirement for Community Based Outpatient Clinic coverage for primary care services in Caledonia, Essex, and Orleans Counties in Vermont, and Coos, and Northern Grafton (North of Haverhill) Counties in New Hampshire. The contractor shall provide for the delivery and management of continuous primary and preventive medical care for enrolled VA patients. This is an RFP, contractor shall address the evaluation criteria listed in the solicitation document. The contractor's proposal shall address one or more options for providing such primary care utilizing contractor provided space, medical equipment and supplies, medical personnel, laboratory and x-ray services, administrative services, and/or a combination(s) of VA provided services along with contractor provided services. Physicians and technicians operating under the resulting contract must meet all evaluation criteria and other professional certifications required to operate in such a clinic. The Government anticipates awarding one or more contracts under this solicitation based on what is most advantageous to the Government.
