RFP 670-04-00 is issued to provide continued primary care services at a location in the County of Jefferson in the State of New York. The Veterans Affairs Healthcare Network Upstate New York (VAHNUNY) desires to continue a contract for Community Based Primary/Preventative Care services including Mental Health services for veterans in this geographic region which has been served by this mechanism since 1997. The resulting contract will be for a period of one year with two options to commence on or about July 1, 2000. The Contractor will be responsible to furnish medical facilities, clinical providers, equipment, supplies, routine radiology and administrative functions to fulfill the level of care specified in the RFP.
