combined synopsis/solicitation for commercial items prepared in
accordance with the format in Federal Acquisition Regulation (FAR) sub
part 12.6 as supplemented with additional information in this notice.
This announcement constitutes the only solicitation; proposals are
being requested and a written solicitation will not be issued.
Solicitation 671-23Q-00 is issued as a request for quotations, (RFQ).
Solicitation documents and incorporated provisions and clauses are
those in effect through Federal Acquisition Circular 97-15. This action
is unrestricted. Standard Industrial Classification Code: 5047. These
supplies meet the criteria for a commercial item as defined in FAR Part
2, and simplified acquisition procedures will be used in accordance
with Federal Acquisition Regulation Part 13 Simplified Acquisition
Procedures. The South Texas Veterans Health Care System, Audie L.
Murphy Division contemplates negotiation on a sole source basis for
SARNS Modular Perfusion System 8000 consisting of the following
components (1) Base 5-Unit, 115V, (5) Roller Pump, 115V, (1) Safety
Monitor, (1) Level Sensor Pads (100/pk), (1) Ultrasonic Gel, (1) Air
Sensor (3/8 x 3/32), (2) Battery Module 115V, (1) Lamp System 8000, (1)
Crossbar 5 Unit Base, (1) 4 Foot Pole, (1) SVCSP 14630 MDX VCM 8000,
(1) SVCSP 15400 Modified for 8K, (2) Fitted Crossbar 8K. The Federal
Acquisition Regulation (FAR) provision 52.212-1, Instructions to
Offerors -- Commercial Items, applies to this acquisition and is
incorporated by reference herein. The FAR provision 52.212-2
Evaluation-Commercial Items applies to this acquisition and is
incorporated by reference herein. Offers will be evaluated on the
lowest overall price to the government. Offerors must include (1) a
completed copy of the provision at FAR 52.212-3, Offeror
Representations and Certifications -- Commercial Items, (2) Contractor
Establishment Code (DUNS Number) and Tax Identification Number.
Veterans Administration Acquisition provision 852.219-70, Veteran-Owned
Small Business applies and is included as an addenda to 52.212-3.
