Dydak, 703-607-6909 Duration: 05/27/2000 through 05/26/2001 and four option years; <The Defense Information Systems Agency (DISA) intends to award a sole source contract for Employee Assistance Program (EAP) couseling services for the headquarters and remote locations. The intended sole source for this requirement is Behavioral Factors at 34 Manatee Way, Crawford, FL 32327. The contractor shall be responsible for providing 8 hours on-site counseling services each week, maintaining case files, and providing education and prevention training sessions to supervisory and non-supervisory staff. Using Behavioral Factors Inc. will prevent disruption of existing cases and maintain client confidentiality. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement consitutes the only solicitation; a proposal is being requested from Behavioral Factors and a written solicitation will not be issued. Request for Proposal DCA100-00-T-4024 applies to this action. The RFP and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-15. The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items applies to this acquisition. The offeror shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The priced proposal must be complete and accompanied by supporting materials, information other than cost or pricing data, which substantiate the proposed price. The clause at Far 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items applies to this acquisition. This will be a firm fixed price contract, and will commence on 27 May 2000. The contract will consist of a base year and four (4) one year option periods, plus the period afforded under FAR 52.217-8.
