This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes a formal Request for Proposal (RFP) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 97-15. RFP 621-13-00 should be referenced on all proposals. The Standard Industrial Classification (SIC) for this procurement is 8099 and the small business size-standard is gross annual receipts NTE $5.0 million. This procurement is unrestricted. The VA Medical Center intends to solicit proposals and award a firm fixed price contract under the authority of Title 38 USC Section 8153, pending availability of funds. The James H. Quillen VA Medical Center, Mountain Home, Tennessee has an emergentneed for a Locum Tenens physician assistant/nurse practitioner (PA/NP) to provide Primary Care services to veteran patients. The PA/NP shall provide Primary Care Clinic coverage approximately 25 to 30 hours per week Monday through Friday, 8:00 a.m. -- 4:30 p.m., excluding Federal Holidays for a period of 60 days with option to extend. No payment will be made for the scheduled half-hour lunch break. On-call coverage WILL NOT be required. Scope of practice will involve the following services: (a) screening outpatients to determine need for further healthcare and enroll in primary care; (b) complete a vesting physical exam on each patient; (c) ordering clinical and diagnostic studies (i.e., lab tests, X-ray, EKG, CT scan, MRI, Arthrogram IVP, ultrasound studies); (d) draw blood specimens for testing and performing other comparable laboratory procedures, as needed; (e) initiating and expediting requests for consultations and scheduling special tests and studies; (f) recording appropriate documentation on all procedures performed; (g) informing and counseling patients in order to obtain informed consent for diagnostic procedures; (h) carry-out health promotion and disease prevention activities for patients, families and the community. PA/NP must be proficient in utilizing CPRS electronic medical record systems. PA/NP duties shall be performed within the parameters of the clinic schedule. Approximately 8-10 outpatients are seen daily. The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for their personnel: worker's compensation, professional liability insurance, health examinations, and income tax withholding and social security payments. The parties agree that such personnel shall not be considered a VA employee for any purpose and shall be considered employees of the contractor. Payment for any leave, including sick leave, holiday or vacation time is the responsibility of the contractor. Offerors may propose more than one candidate. Candidate must be available for immediate duty. Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. The government intends to evaluate offers and award without discussions. However, the government reserves the right to conduct discussions later if determined necessary by the Contracting officer.
