combined synopsis/solicitation for commercial items prepared in
accordance with the format in FAR Subpart 12.6, as supplemented with
additional information included in this notice. This solicitation
DADA13-00-T-0060 is issued as a Request for Quotation. This
announcement constitutes the only solicitation. Quotations are being
requested and a written solicitation will not be issued. This
solicitation document and incorporated clauses/provisions are those in
effect through Federal Acquisition Circular 97-15. SIC Code is 5047and
the small business standard is 500 employees. This requirement is set
aside for small business. The Western Regional Contracting Office
intends to award a firm fixed price contract. The vendor shall provide
the following: CLIN 0001: (This CLIN is for informational purposes
only) Vascular-Tattoo Laser Systems Package consists of the following:
1 Vascular Pulse Dye type Laser; 1 Cooling Device; 1 Tattoo
Alexandrite type Laser; 3mm Handpiece w/aiming guide and six (6)
disposable distance gauges; 2 each operator's manuals; 2 each service
maintenance manuals; Safety glasses; Installation; On-site In Service;
Warranty that includes travel, labor and non-consumable parts. CLIN
000101: Vascular Pulse Dye Type Laser Minimum Specifications-Shall
consist of appropriate variable wavelengths to be compatible for use in
both adult and children for vascular malformations specifically port
wine stains; Shall have cooling system to allow for skin cooling before
laser energy is deposited; Shall have a pulse time long enough (1500
usec) to deliver laser energy over time; Shall have variable spot sizes
up to 10mm; Automatic energy setting verification monitoring is a major
attribute; Electrical requirements 100/230 VAC, 50/60 Hz; Pulse control
via foot switching and finger switching devices; Shall include seven
(7) pairs of safety glasses (goggles, spectacles, etc.); Shall include
cooling agent as required by cooling system; Shall include appropriate
accessory kits for routine operation of the system; Shall include two
(2) copies each operating instructions, maintenance instructions, and
schematic drawings with requested items of equipment; CLIN 000102:
Tattoo Alexandrite Type Laser Minimum Specifications-Shall be able to
treat multicolored tattoos and pigmented lesions; Shall have a
q-switched mode; Shall have a short pulse duration of less than 80
nanoseconds; Shall have multiple spot sizes up to 4mm; Pulse control
via foot switching and finger switching devices; Shall include seven
(7) pairs of safety glasses (goggles, spectacles, etc.); Shall include
appropriate accessory kits for routine operation of the system; Shall
include two (2) copies each of operating instructions, maintenance
instructions, and schematic drawings with requested items of equipment.

52.212-2(a) and will be used to evaluate offers: Technical capability,
Price and Past Performance. Technical capability and price when
combined are significantly more important than Past Performance.

(all offerors are to include with their response a completed copy of
FAR 52.212-3). Further, offers must contain all commercial terms,
conditions and provisions under which the offeror conducts business in
the commercial marketplace, and warranty terms and conditions. Offers

reject the terms and conditions of this solicitation may be excluded

The contractor shall ensure products provided under this contract to
include hardware, software, firmware and middleware, whether acting
alone or combined as a system are Year 2000 compliant as defined in FAR

equivalent format, can be mailed to Western Regional Contracting
Office, Madigan Army Medical Center, ATTN: Wanda Reed, 9902Lincoln
Street, Tacoma, WA 98431. Offers must provide as a minimum: 1) RFQ
number; 2) Name, address, and telephone number of point of contact of
offeror; 3) CLINS 000101 and 000102 Price and discount terms; 4) A
completed copy FAR 52.212-3 Representations and Certifications; 5)
Sufficient technical literature to allow the Contracting Officer to
evaluate; 6) Provide a minimum of two (2) current references for past
performance information to include point of contact name, telephone
number and type of supply for whom contracts have been satisfactorily
completed. Offerors are responsible for accurate addresses and
telephone numbers for submitted past performance information. Basis of
Award will be on evaluation factors related to technical capability,
price and past performance. THIS IS A BEST VALUE AWARD. The Government
reserves the right to award to other than the lowest priced offeror.
You must submit along with your offer, documents, which clearly
describe the equipment, so that it can be evaluated. Equipment not
clearly described will not be considered. Equipment otherwise equal may
not satisfy the requirement due to differences in environmental
